目的 检测心力衰竭患者外周血粒细胞 巨噬细胞集落刺激因子 (GM CSF)、白细胞介素 8(IL 8)及白细胞的水平 ,并探讨他们在心力衰竭中的关系。方法 5 9例心力衰竭患者 ,根据NYHA分级分成心功能Ⅱ、Ⅲ、Ⅳ级 3组 ,测定血清GM CSF及IL 8水平采用放射免疫法 ,并镜检血WBC数。并与年龄、性别相匹配的正常对照组 (19例 )进行比较。结果 心力衰竭组外周血GM CSF、IL 8及WBC水平与正常对照组比较有显著性差异 (P <0 .0 1) ,且随心力衰竭程度的加重而更明显 ;不同病因的心力衰竭患者之间无显著差异 (P >0 .0 5 ) ;心力衰竭患者外周血GM CSF与IL 8及WBC呈显著正相关 (r值分别为 0 .6 0 2 ,0 .5 4 1;P <0 .0 1)。结论 心力衰竭患者外周血GM CSF、IL 8及WBC水平增高 ,并随心衰程度的加重而加重 ;提示炎症反应可能参与了心力衰竭的发生和发展 ,而GM CSF在其中可能起重要作用。
Objective To detect the levels of peripheral blood granulocyte-macrophage colony-stimulating factor (GM CSF), interleukin 8 (IL 8) and leucocytes in patients with heart failure and to explore their relationship in heart failure. Methods Fifty-nine patients with heart failure were divided into three groups according to NYHA class Ⅱ, Ⅲ and Ⅳ. Serum GM CSF and IL 8 levels were measured by radioimmunoassay and the number of blood WBC was examined. And compared with age and sex matched normal control group (19 cases). Results The levels of GM CSF, IL 8 and WBC in peripheral blood of patients with heart failure were significantly higher than those of the normal controls (P <0.01), and were more pronounced with the severity of heart failure. Patients with heart failure of different etiologies There was no significant difference between the two groups (P> 0.05). There was a significant positive correlation between GM CSF and IL-8 and WBC in patients with heart failure (r = 0.60, 0.451, P <0. 0 1). Conclusion The levels of GM CSF, IL 8 and WBC in peripheral blood of patients with heart failure are increased and aggravate with the worsening of heart failure. It is suggested that inflammatory response may be involved in the occurrence and development of heart failure, and GM CSF may play an important role in them.
