目的对由区结防所在社区卫生服务中心内设立的肺结核病防治站(以下简称“防治站”)和社区卫生站聘任的结防督导员(以下简称“督导员”)组成的全区结核病防治网络(以下简称“建网”)在结核病控制工作中的作用进行评价。方法分析朝阳区2002年1月—2007年12月登记的肺结核病例资料,对建网前3年(2002年1月—2004年12月)与建网后3年(2005年1月—2007年12月)肺结核的病例发现、户籍人口和流动人口病例的DOTS管理和涂阳病例的治疗转归等情况进行比较。结果建网前后比较:(1)病例发现由789例增至1 322例,增加了533例(67.6%)。防治站发现病例占登记病例总数的40%以上。(2)涂阳病例DOTS管理率平均由63.0%提高到92.9%。(3)涂阳病例治疗成功率户籍人口平均由83.3%提高到92.4%,流动人口平均由65.5%提高到80.7%。(4)建网后防治站与区结防所的涂阳病例治疗成功率比较无差异。结论在加强政府承诺的同时,建立地区结核病防治网络是解决结核病控制工作中的困难和提高质量的重要措施。
Aims To establish a tuberculosis prevention and control station (hereinafter referred to as “prevention and control station”) and an endorser (hereinafter referred to as “supervisor”) appointed by a community health station in the community health service center where the district prevention and control center is located, The TB control network in the region (hereinafter referred to as “building network ”) in the role of tuberculosis control evaluation. Methods The data of tuberculosis cases registered in Chaoyang District from January 2002 to December 2007 were analyzed. The data of tuberculosis cases registered in the first three years of construction (January 2002 to December 2004) and three years after the establishment of the network (January 2005 to 2007 December) case finding of tuberculosis, DOTS management of census register population and floating population cases and treatment outcome of smear positive cases were compared. Results Before and after the establishment of the network: (1) The number of cases was increased from 789 to 1 322 cases, an increase of 533 cases (67.6%). Control stations found cases accounted for more than 40% of the total number of registered cases. (2) The DOTS management rate of smear positive cases increased from 63.0% to 92.9% on average. (3) The successful treatment rate of smear positive cases The registered population increased from 83.3% to 92.4% on average, and the floating population increased from 65.5% to 80.7% on average. (4) There was no difference in the success rate of smear-positive cases between prevention and cure stations and area prevention and cure stations after the network construction. Conclusions While strengthening the government’s commitment, setting up a regional network for tuberculosis prevention and control is an important measure to solve the difficulties and improve the quality of tuberculosis control.
