目的评价重型颅脑损伤(SHI)患者开颅术中低血压对预后的影响。方法回顾性分析263例SHI患者资料。行急诊开颅手术183例,对63例术中低血压患者进行复苏,并对预后进行评估。结果63例中,恢复良好的11例,中度残疾6例,重度残疾2例,植物生存1例,死亡43例,病死率68.3%,高于同期开颅术中无低血压表现者(病死率30.2%)。结论SHI术中出现低血压明显增加患者的病死率,对术中低血压进行积极防治可改善预后。
Objective To evaluate the effect of hypotension during craniotomy on prognosis in patients with severe craniocerebral injury (SHI). Methods The data of 263 SHI patients were retrospectively analyzed. 183 cases of emergency craniotomy, 63 cases of intraoperative hypotensive patients were resuscitated and the prognosis was evaluated. Results In 63 cases, 11 cases were recovered well, 6 cases were moderate disability, 2 cases were severe disability, 1 case was plant survival, 43 cases died and the case fatality rate was 68.3%, which was higher than those without hypotension during the same period of craniotomy Rate of 30.2%). Conclusions The hypotension in patients with SHI significantly increases the mortality of patients, and the active prevention and treatment of intraoperative hypotension can improve the prognosis.
