目的了解IFG与T2DM患者血清瘦素和胰升血糖素水平的临床特点,初步研究瘦素和胰升血糖素与IR的关系。方法选取22例IFG者,22例新诊断的T2DM患者和48名正常对照(NC)者,分别测定BMI、FPG、TG、FIns、瘦素、胰升血糖素,按公式计算胰岛素抵抗指数[HOMA-IR=FIns(mU/L)×FPG(mmol/L)/22.5]。结果 IFG与T2DM患者同NC组相比瘦素、胰升血糖素水平明显增高(P<0.05),分别是[(5.86±1.71)、(5.73±1.68)、(4.83±1.82)]ng/ml,[(246.54±32.75)、(278.37±42.56)、(208.76±38.65)]pg/ml;多因素分析,IFG与T2DM患者的瘦素水平与BMI、HOMA-IR呈正相关(r=0.58,0.61,P<0.05);IFG与T2DM患者的胰升血糖素水平与HOMA-IR呈正相关(r=0.54,P<0.05)。结论(1)IFG与T2DM患者的瘦素和胰升血糖素比NC者高。(2)T2DM患者的瘦素水平与BMI、HOMA-IR呈正相关;IFG与T2DM患者的胰升血糖素水平与HOMA-IR呈正相关。
Objective To investigate the clinical features of serum leptin and glucagon levels in patients with IFG and T2DM and to study the relationship between leptin and glucagon and IR. Methods Twenty-two patients with IFG, 22 patients with newly diagnosed T2DM and 48 normal controls were enrolled in this study. BMI, FPG, TG, FIns, leptin and glucagon were measured respectively. The insulin resistance index [HOMA -IR = FIns (mU / L) × FPG (mmol / L) /22.5]. Results The levels of leptin and glucagon in IFG and T2DM patients were significantly higher than those in NC group (P <0.05), which were (5.86 ± 1.71), (5.73 ± 1.68) and (4.83 ± 1.82) ng / ml , (246.54 ± 32.75), (278.37 ± 42.56) and (208.76 ± 38.65) pg / ml respectively. Multivariate analysis showed that the leptin level was positively correlated with BMI and HOMA-IR in IFG and T2DM patients (r = 0.58 and 0.61 , P <0.05). The levels of glucagon in IFG and T2DM patients were positively correlated with HOMA-IR (r = 0.54, P <0.05). Conclusions (1) The levels of leptin and glucagon in patients with IFG and T2DM are higher than those with NC. (2) Leptin levels in T2DM patients were positively correlated with BMI and HOMA-IR. Glucagon levels in patients with IFG and T2DM were positively correlated with HOMA-IR.
