目的:探讨磁性附着体与改良杆卡式附着体两种义齿修复单侧游离缺失对支持组织应力分布的影响。方法:分别以磁性、改良杆卡式两种固位形式的附着体义齿修复下颌单侧游离缺失,用三维光弹应力冻结切片技术测试义齿在垂直力、侧向力加载时对基牙和缺牙区牙槽骨应力分布大小。结果:①两种义齿对基牙牙槽骨的应力比较:磁性附着体垂直力明显大于磁性附着体侧向力(P<0.01),磁性附着体垂直力与改良杆卡式附着体垂直力无差别(P>0.05),磁性附着体侧向力小于改良杆卡式附着体侧向力(P<0.05);改良杆卡式附着体垂直力大于改良杆卡式附着体侧向力(P<0.05)。②两种义齿对缺牙区牙槽骨应力比较:磁性附着体垂直力明显大于磁性附着体侧向力(P<0.01),磁性附着体垂直力与改良杆卡式附着体垂直力无差别(P>0.05),磁性附着体侧向力小于改良杆卡式附着体侧向力(P<0.05);改良杆卡式附着体垂直力与改良杆卡式附着体侧向力无差别(P>0.05)。结论:磁固位附着体与改良杆卡式附着体两种义齿都能达到较好的修复效果,垂直力时两种义齿对观测点的应力分布较均匀,并且两种义齿无差别;侧向力的磁性附着体可发生相对移位形成应力中断,相对于杆卡式附着体能够达到更好的应力分布,可以明显降低基牙周围骨组织的应力。
OBJECTIVE: To investigate the effect of two types of dentures with magnetic attachment and modified rod attachment on the stress distribution in the supporting tissue. Methods: The mandibular unilateral mandibular free-standing denture was fixed by two kinds of rehabilitated rod-shaped magnetic attachments and the modified rod-shaped denture. Three-dimensional photoelastic stress freeze-thawing technique was used to test the effect of vertical force and lateral force on abutment and absent Tooth alveolar bone stress distribution size. Results: ① Comparison of stress between the two kinds of dentures on alveolar bone: the vertical force of magnetic attachments was significantly greater than that of magnetic attachments (P <0.01), and there was no difference between the vertical force of magnetic attachments and the vertical force of modified rod attachments (P <0.05). The lateral force of magnetic attachment was less than that of modified card attachment (P <0.05). The vertical force of modified card attachment was greater than that of modified card attachment ). (2) The stress of alveolar bone in two kinds of denture were compared: the vertical force of magnetic attachment was obviously larger than that of magnetic attachment (P <0.01), and there was no difference between the vertical force of magnetic attachment and the vertical force of modified card attachment P> 0.05). The lateral force of the magnetic attachment was less than that of the modified rod attachment (P <0.05). There was no difference between the vertical force of the modified rod attachment and that of the modified rod attachment (P> 0.05). Conclusion: The two kinds of dentures with magnetic retention attachment and modified attachment can achieve better repair effect. The stress distribution of the two kinds of denture to the observation point is more uniform when the vertical force is applied, and there is no difference between the two dentures. The force of magnetic attachment can occur relative displacement of the stress interrupt, compared with the rod clip attachment can achieve better stress distribution, can significantly reduce the stress around the abutment bone tissue.
