无创血流动力学监测在硝普钠治疗急性心力衰竭中的应用

目的:评价无创血流动力学监测系统在硝普钠治疗急性心力衰竭中的应用价值。方法:选择急性心力衰竭病人32例,在常规治疗基础上用微泵静推硝普钠,疗程1~2d。用香港千帆医疗仪器有限公司CSM3000无创血流动力学检测系统进行持续监测,记录给药前及用药24h后的血流动力学参数,并选取其中7种:收缩压(SBP)、舒张压(DBP)、心率(HR)、心排血量(CO)、心脏指数(CI)、每搏量(SV)、射血分数(LVEF)、收缩时间比率(STR)做血流动力学定量评估。结果:硝普钠治疗各种原因所致的急性心力衰竭均有显著效果,收缩压、舒张压、心率、收缩时间比率下降,心排出量、每搏量、左室射血分数升高,分别同用药前比较均有显著性差异(P<0.01)。结论:CSM3000无创血流动力学监测系统临床使用方便,重复性好,能连续检测观察病情变化,及时反映急性心力衰竭患者治疗过程中的病情变化,具有较强的实用价值。 Objective: To evaluate the value of noninvasive hemodynamic monitoring system in the treatment of acute heart failure with sodium nitroprusside. Methods: Thirty-two patients with acute heart failure were selected. Sodium nitroprusside was given by micro-pump on the basis of routine treatment. The course of treatment was 1-2 days. Continuous monitoring was performed with CSM3000 non-invasive hemodynamic test system of Qianfan Medical Instrument Co., Ltd. Hong Kong, and the hemodynamic parameters were recorded before and 24 hours after treatment. Seven of them were selected: systolic blood pressure (SBP), diastolic blood pressure (DBP ), Heart rate (HR), cardiac output (CO), cardiac index (CI), stroke volume (SV), ejection fraction (LVEF) and systolic time (STR) Results: Sodium nitroprusside treatment of acute heart failure caused by various causes have a significant effect, systolic blood pressure, diastolic blood pressure, heart rate, decreased rate of contraction time, cardiac output, stroke volume, left ventricular ejection fraction increased Compared with before treatment were significant differences (P <0.01). Conclusion: CSM3000 noninvasive hemodynamic monitoring system is easy to use and has good repeatability. It can continuously detect the changes of the disease and reflect the changes of the disease in the course of treatment in patients with acute heart failure. It has strong practical value.
