Nilvadipine(N)是一种新型二氢吡啶衍生物,其化学结构很相似于硝苯吡啶,是有效的血管扩张剂。本文评价了N对充血性心力衰竭患者中心和外周循环的急性血液动力学作用。 12例充血性心衰病人,平均年龄59岁(44~84岁),其中扩张性心肌病4例;二尖瓣返流3例,主动脉返流3例,高血压心脏病2例。给药前24小时停止除洋地黄外的一切药物治疗。试验在安静温暖的房间里进行,先休息30分钟,然后测动脉血压。9例作右心导管检查,测量右房、肺毛细血管楔压及肺动脉压;用热稀释法测量心输出量。9例则用常规静脉闭塞技术测量前臂静脉压和血容量的改变。随即口服N6mg粉剂后,于15、30、60分钟各重复同样测量。
Nilvadipine (N) is a novel dihydropyridine derivative whose chemical structure is very similar to nifedipine and is a potent vasodilator. This article assesses the acute hemodynamic effects of N on central and peripheral circulation in patients with congestive heart failure. 12 patients with congestive heart failure, mean age 59 years (44 to 84 years), including dilated cardiomyopathy in 4 cases; 3 cases of mitral regurgitation, aortic regurgitation in 3 cases, 2 cases of hypertensive heart disease. 24 hours prior to administration of all drugs except digitalis stop treatment. The test was performed in a quiet and warm room, first resting for 30 minutes and then measuring arterial blood pressure. 9 cases of right heart catheterization, measurement of right atrium, pulmonary capillary wedge pressure and pulmonary artery pressure; measured by cardiac output with thermal dilution method. 9 cases were measured using conventional venous occlusion forearm venous pressure and blood volume changes. Immediately after oral administration of N6mg powder, the same measurements were repeated at 15,30,60 minutes each.
