慢性粒细胞白血病骨髓移植治疗进展

慢性粒细胞白血病(CML)是造血系统的一种恶性增殖性疾病,临床起先表现为一个较长的慢性期(CP),1~5年后大多进展为急变期,使用马利兰及羟基脲作常规化疗可控制症状和体征,但不能延缓急变的发生及延长患者生长期。异基因骨髓移植是目前唯一能够治愈CML方法。但绝大多数患者缺乏配型相合的同胞供髓者。使用无关供髓者的骨髓移植,机会较多,但移植相关性疾病发病率高,并往往致死。由于体外骨髓净化技术的提高,体外可去除CML克隆,自体骨髓移植近年来受到一定重视。本文就骨髓移植在CML治疗中的一些进展作一综述。 Chronic myelogenous leukemia (CML) is a malignant proliferative disease of the hematopoietic system. The clinical manifestation of chronic myelogenous leukemia (CML) is a relatively long-term chronic phase (CP). After 1 to 5 years, most of them develop into a blast crisis. Marelan and hydroxyurea are used routinely. Chemotherapy can control symptoms and signs, but it can not delay the occurrence of rapid changes and prolong the growth period of patients. Allogeneic bone marrow transplantation is currently the only method that can cure CML. However, the vast majority of patients lack matched sibling donors. Bone marrow transplantation using unrelated donors is more likely to occur, but the incidence of transplant-related diseases is high and often leads to death. Due to the improvement of bone marrow purification techniques in vitro, CML clones can be removed in vitro. Autologous bone marrow transplantation has received some attention in recent years. This article reviews the progress of bone marrow transplantation in the treatment of CML.
