目的:探讨1例稀有血型结肠癌患者,在紧急抢救输血时因存在不规则抗体,导致交叉配血困难,而采取特殊情况紧急抢救输血方案的可行性。方法:首次采用全自动血型仪(微孔板法)检测患者血型,并用试管法复核;不规则抗体筛查及鉴定抗体筛查采用盐水法和微柱凝胶卡法;交叉配血试验采用盐水法、聚凝胺法、间接抗球蛋白法(试管法)。结果:患者血型为A型,Rh表型为cc DEE型;从已有备库细胞中的O型供者中找到cc DEE悬浮红细胞1 U,并从广东省血液中心的A型供者中筛查到cc DEE悬浮红细胞3 U,输注后12 h检测血红蛋白61 g/L,提示输注有效。结论:对于因不规则抗体存在,导致配血困难而又急需输血抢救的肿瘤患者,在ABO血型同型无法满足供应时,可筛选O型Rh表型一致且交叉配血试验阴性的供者红细胞输注。
Objective: To investigate the feasibility of emergency rescue of blood transfusion in a special case of patients with rare blood group colon cancer due to the presence of irregular antibodies in the emergency rescue of blood transfusion resulting in cross-matching of blood. Methods: The blood type of patients was detected by automatic blood type instrument (microplate method) for the first time, and the blood type was examined by test tube method. One cc of CED DEE was found from O-donors in stocked cells and screened from Type A donors at Guangdong Provincial Blood Center Check the cc DEE suspended 3 U, 12 h after infusion hemoglobin detection 61 g / L, suggesting that infusion is effective. CONCLUSIONS: For patients with tumors that are irregularly antibody-resistant and require transfusion urgently due to irregular antibodies, they can screen donor OBCs whose O phenotypic Rh phenotype is consistent and crossmatch negative Note.
