目的探讨胃黏膜PH值对小儿重症肺炎合并胃黏膜损伤的诊断价值。方法选取我院2008年6月至2011年6月196例重症监护患儿,其中有104例患儿发生肺炎,84例为出现肺炎,分析其临床症状,测定患者胃黏膜PH值,检测病菌类型及胃黏膜的损伤程度,统计分析胃黏膜PH值和病原菌引发小儿重症肺炎的关系。结果 196例患儿当中,发生肺炎几率是53.1%。胃液pH值对胃腔中定植菌种类产生一定的影响,当胃液pH值≤4,胃腔中的定植菌主要有念珠菌及革兰阳性杆菌;胃黏膜PH值>4,胃腔中检出革兰阴性杆菌几率明显增加。胃黏膜pH值≤4时,肺炎发生几率是34.5%;pH值>4时,肺炎发生率达到60.9%。pH值>4时肺炎发生率明显高于pH值≤4时肺炎发生率,差异有统计学意义(P<0.05)。结论胃黏膜pH值影响胃腔定植菌,胃液pH值增高,肺炎发生几率越大,对小儿重症肺炎合并胃黏膜损伤的诊断有较高的价值。
Objective To investigate the value of gastric mucosal PH value in the diagnosis of severe pneumonia complicated with gastric mucosal injury in children. Methods A total of 196 intensive care patients from June 2008 to June 2011 in our hospital were selected. Among them, 104 cases developed pneumonia and 84 cases were pneumonia. The incidence of pneumonia was 34.5% when pH of gastric mucosa was less than 4, and the incidence of pneumonia was 60.9% when pH value was> 4. The incidence of pneumonia at pH> 4 was significantly higher than that at pH ≤4 (P <0.05). Conclusion The gastric mucosal pH value affects the colonization of gastric cavity, the pH value of gastric juice increased, the greater the risk of pneumonia, the diagnosis of severe pneumonia in children with gastric mucosal injury have a higher value.
