目的:探讨急性粒单核细胞白血病(M4)和急性单核细胞白血病(M5)的核型与预后的关系。方法:采用直接法和短期培养法制备骨髓细胞染色体,以R显带技术,对50例M4/M5患者进行染色体核型分析。结果:50例M4/M5中,24例(48.0%)检出核型异常。其中累及8号染色体异常(除外复杂核型)9例,3号染色体异常4例,复杂核型异常9例,7号及11号染色体异常各1例。化疗后9例获得完全缓解,缓解率37.5%(9/24),明显低于正常核型的缓解率65.4%(17/26),差异有显著性(χ2=3.89,P<0.05)。结论:核型异常的M4/M5患者,特别是3号染色体异常和复杂核型异常组,疗效与预后较差。
Objective: To investigate the relationship between karyotype and prognosis of acute myelomonocytic leukemia (M4) and acute monocytic leukemia (M5). Methods: Chromosomes of bone marrow cells were prepared by direct method and short-term culture method. Chromosome karyotype analysis was performed on 50 M4 / M5 patients by R banding technique. Results: In 50 cases of M4 / M5, 24 cases (48.0%) detected karyotype abnormalities. There were 9 cases with abnormal chromosome 8 (except complex karyotype), 4 cases with abnormal chromosome 3, 9 cases with complicated karyotype, and 1 case with abnormal chromosome 7 and 11. After chemotherapy, complete remission was achieved in 9 cases, with a remission rate of 37.5% (9/24), which was significantly lower than that of normal karyotype (65.4%, 17/26) (χ2 = 3.89, P <0.05). Conclusions: The efficacy and prognosis of M4 / M5 patients with abnormal karyotypes, especially chromosome 3 abnormalities and complex karyotype abnormalities, are poor.
