目的探讨高龄初产妇对妊娠结局的影响。方法对2009年1月至2011年9月收治的高龄初产妇的妊娠并发症发生率、分娩方式及围生儿情况等进行回顾性分析。结果观察组剖宫产率为90.91%,明显高于对照组的43.64%;产妇妊娠期合并高血压及糖尿病的例数观察组明显高于对照组,差异有统计学意义(P<0.05);而产妇巨大儿及胎盘早剥发生率两组无统计学意义(P>0.05);胎儿宫内窘迫发生率对照组高于观察组,差异有统计学意义(P<0.05);胎膜早破、早产儿、产后出血发生率观察组高于对照组,差异有统计学意义(P<0.05)。结论高龄初产妇妊娠合并症和并发症的发生率明显高于非高龄初产妇,因此应加强高龄孕产妇的围产期保健,预防妊娠并发症,合理控制剖宫产率,以确保母婴安全与健康。
Objective To investigate the effect of early primipara on pregnancy outcome. Methods Retrospective analysis was made on the incidence of pregnancy complications, mode of delivery and perinatal conditions of the elderly primipara from January 2009 to September 2011. Results The rate of cesarean section in the observation group was 90.91%, which was significantly higher than that in the control group (43.64%). The number of cases of pregnancy complicated with hypertension and diabetes was significantly higher in the observation group than in the control group (P <0.05). The incidence of maternal macrosomia and placental abruption had no statistical significance (P> 0.05). The incidence of fetal distress was higher in the control group than in the observation group (P <0.05). Premature rupture of membranes , Preterm children, the incidence of postpartum hemorrhage observation group was higher than the control group, the difference was statistically significant (P <0.05). Conclusion The incidence of complications and complications of primipara in early pregnancy is obviously higher than that of non-adolescent primipara, so we should strengthen the perinatal care of the elderly pregnant women, prevent pregnancy complications and control the rate of cesarean section reasonably to ensure the safety of mother and baby And health.
