目的:探讨高迁移率族蛋白B1(HMGB1)在胃肠间质瘤(GISTs)组织中的表达及临床病理意义。方法:收集东阳市人民医院病理科2006年1月至2019年6月检查的GISTs 组织石蜡标本130例,采用免疫组织化学EnVision法检测GISTs组织中HMGB1的表达,分析HMGB1胞核高表达及胞质阳性表达与GISTs患者临床病理特征的关系。结果:GISTs组织中HMGB1胞核高表达率、胞质阳性率分别为75.4%(98/130)、23.8%(31/130)。在肠道及肿块>5 cm的GISTs患者中,其HMGB1胞质阳性率分别显著高于非肠道及肿块≤5 cm者(χn 2=6.672、14.412,n P=0.036、5个/50 HPF的GISTs中有增强(35.3%,12/34),但差异无统计学意义(χn 2=3.323,n P=0.068)。HMGB1胞质阳性率在极低危、低危、中危、高危的GISTs组织中分别为0.0%(0/15)、12.2%(5/36)、24.1%(7/22)、42.2%(19/26),呈逐步增强趋势,差异有统计学意义(χn 2=16.131,n P=0.001)。HMGB1胞核高表达与GISTs患者各项临床病理特征均无关(均n P>0.05)。n 结论:HMGB1胞质阳性表达与GISTs患者多项预后不良因素及危险度分级密切相关,提示其高表达与GISTs患者发生及进展密切相关。“,”Objective:To investigate the expression of high mobility group box 1 (HMGB1) in gastrointestinal stromal tumors (GISTs) and its clinical pathological significance.Methods:GISIs paraffin specimens of 130 patients who received treatment in the Department of Pathology, Dongyang People's Hospital from January 2006 to June 2019 were included in this study. HMGB1 expression in GISTs was detected using immunohistochemical EnVision method. High expression of HMGB1 in the nucleus and positive expression of HMGB1 in the cytoplasm were correlated with the clinical pathological characteristics of GISTs.Results:The positive rate of HMGB1 in the nucleus and cytoplasm was 75.4% (98/130) and 23.8% (31/130), respectively. Cytoplasmic positive rate of HMGB1 in patients with GISTs ( > 5 cm lump) locating in the intestinal tract was significantly higher than that in patients with GISTs ( ≤ 5 cm lump) locating not in the intestinal tract (χ n 2= 6.672,14.412,n P = 0.036, 5/50× high power field was slightly,but not significantly,higher than that in GISTs patients with mitotic counts ≤ 5/50× high power field [35.3% (12/34) n vs. 19.8% (19/96), χn 2= 3.323,n P = 0.068]. Cytoplasmic positive rate of HMGB1 in the extremely low-risk, low-risk, medium-risk, and high-risk GISTs was 0.0% (0/15), 12.2% (5/36),24.1% (7/22) and 42.2% (19/26), respectively, showing a gradual increasing trend.The difference between risk grades was significant (χn 2= 16.131, n P = 0.001). High expression of HMGB1 in the nucleus was not related to the various clinical pathological characteristics of patients with GISTs (all n P > 0.05).n Conclusion:Cytoplasmic positive expression of HMGB1 is closely related to a variety of poor prognosis factors and risk grades of GISTs, suggesting that high cytoplasmic expression of HMGB1 is closely related to the occurrence and progression of GISTs.
