目的:研究血清同型半胱氨酸(homocysteine,Hcy)、基质金属蛋白酶-9(matrix meta11opro teinases-9,MMP-9)及颈动脉内-中膜厚度(carotid intima-media thickness,IMT)在冠心病中的诊断价值。方法以急性冠脉综合征(ACS)40例、稳定型心绞痛组(SA)22例并行冠状动脉造影检查的患者为观察组,同时以健康者28例为对照组,测定其Hcy、MMP-9浓度和颈动脉IMT厚度。结果 ACS组、SA组血清Hcy、MMP-9及颈动脉IMT水平均明显高于对照组,且ACS组血清Hcy、MMP-9水平明显高于SA组(P<0.05);多支病变组血清Hcy 、MMP-9浓度高于单支病变组(P<0.05);冠心病患者血清Hcy、MMP-9及颈动脉IMT三者间有明显的相关性(r分别为0.351、0.573、0.612,P<0.05)。结论冠心病患者血清Hcy、MMP-9及颈动脉IMT水平明显升高;血清Hcy、MMP-9可在一定程度上反映粥样斑块的不稳定性且与冠脉病变的范围相关,联合检测三者对冠心病的诊断及治疗有着重要的意义。“,”Objective To study the value of the serum homocysteine (homocysteine, Hcy), matrix metalloproteinase -9 (matrix meta11opro teinases-9, MMP-9) and carotid artery- media thickness (carotid the intima - media thickness, IMT) in the diagnosis of coronary heart disease. Methods To choose 40 cases of acute coronary syndrome patients (ACS), 22 cases of stable angina (SA) as the observation group, and to choose 28 cases of healthy persons as normal control group during the same period and measur the serum of Hcy, MMP-9 content and thickness of carotid IMT. Results The serum Hcy, MMP-9 and carotid artery IMT levels in ACS group and SA group were significantly higher than those in control group;Serum Hcy, MMP-9 levels of ACS group were higher than those in SA group ( P< 0.05 );The serum Hcy, MMP-9 level of multivessel disease group were higher than that of single vessel disease group (P<0.05 );There was a significant correlation between serum Hcy, MMP-9 level and carotid artery IMT of patients with coronary heart disease ( r were 0.351, 0.573, 0.612, P<0.05 ). Conclusion The serum Hcy, MMP-9 and IMT levels are significantly elevated in carotid artery; Serum Hcy, MMP-9 level can some extent reflect the atherosclerotic plaque instability and are related to coronary artery lesio range, There is great significance to detect them together in coronary heart disease diagnosis and treatment.
