目的定量检测三碘甲状腺原氨酸(T_3)、游离T_3(FT_3)、肌钙蛋白I(cTnI)、肌红蛋白(Mb)以了解冠心病患者T_3、FT_3,是否有随病情加重而变化。方法A组:正常对照组50人,于早晨空腹取静脉血3ml送检;B组:冠心病患者无胸痛症状者107人,男68人,女39人,于早晨空腹取静脉血3ml送检;C组:冠心病患者有胸痛症状者58人,其中男39人,女19人,于胸痛后3h取静脉血3ml送检;A、B、C组全部采用全自动微粒子化学发光分析仪定量检测T_3、FT_3、cTnI、Mb。结果A组T_3浓度为2.08±0.62nmol/L、FT_3浓度为4.9±0.97nmol/L,cTnI浓度为0.27±0.18ug/L,Mb浓度为42±21ug/L;B组T_3浓度为1.68±0.43nmol/L、FT_3浓度为3.25±2.54nmol/L,cTnI浓度为0.31±0.19ug/L,Mb浓度为47±27ug/L,B有3例cTnI Mb显著增高,诊断为无痛性急性心肌梗死;C组T_3浓度为1.29±0.52nmol/L、FT_3浓度为2.93±1.12nmol/L、cTnI浓度为16.47±6.23ug/L,Mb浓度为496.84±253.12ug/L;C组与A组和B组比较有显著差异。结论对无痛性AMI患者何时检测cTnI Mb比较困难,目前尚无预测指标,T_3、FT_3的变化对无痛性AMI有提示作用。对有胸痛症状的冠心病患者检测cTnI Mb可早期诊断AMI,及时溶栓和治疗。
Objective To quantitatively detect the levels of T_3, FT_3, cTnI and Mb in coronary heart disease (CHD) patients to find out whether T_3 and FT_3 in patients with coronary heart disease are changed with the progression of the disease. Methods Group A: normal control group of 50 people in the morning fasting venous blood 3ml for inspection; B group: coronary heart disease patients without chest pain symptoms 107 people, 68 males and 39 females, in the morning fasting venous blood 3ml submission ; Group C: 58 patients with chest pain in CHD patients, including 39 males and 19 females, were given 3ml venous blood 3h after chest pain. Conclusion It is difficult to detect cTnI Mb in patients with painless AMI. There are no predictive indexes at present. The changes of T_3 and FT_3 are indicative of painless AMI. Coronary heart disease patients with chest pain detection of cTnI Mb early diagnosis of AMI, timely thrombolysis and treatment.
