目的 探讨老年 2型糖尿病患者脂代谢紊乱程度是否重于其他非老年 2型糖尿病患者。方法 对 12 8例老年 2型糖尿病、5 9例非老年 2型糖尿病及 5 0例健康人群的血脂、脂蛋白及载脂蛋白进行对比分析 ,并对 187例2型糖尿病患者的甘油三酯 (TG)与年龄及病程进行直线相关分析。结果 老年及非老年组 2型糖尿病与对照组比较 :总胆固醇 (TC)、TG、载脂蛋白 (APO)B及极低密度脂蛋白 (VLDL)均有明显升高 ,而高密度脂蛋白 (HDL)均有下降 ,其P值 <0 .0 5、0 .0 1、0 .0 5或 0 .0 0 1。老年组与非老年组比较 :非老年组TG明显高于老年组 (P <0 .0 0 5 ) ,而HDL却低于老年组 (P <0 .0 5 ) ,且 2型糖尿病患者TG与年龄及病程呈明显负相关 (γ =-0 .2 843 ,γ =-1.3 443 ,P <0 .0 0 1)。结论 2型糖尿病患者普遍存在脂代谢紊乱 ;但中青年 2型糖尿病患者脂代谢异常重于老年 2型糖尿病患者 ,提示更要早期重视中青年 2型糖尿病患者脂代谢紊乱的防治。
Objective To investigate whether the severity of lipid metabolism in elderly patients with type 2 diabetes is more severe than those in other non-elderly type 2 diabetic patients. Methods The blood lipids, lipoproteins and apolipoproteins of 128 cases of type 2 diabetes mellitus, 59 cases of non-elderly type 2 diabetes mellitus and 50 healthy controls were compared and analyzed. The triglyceride TG) and age and course of a linear correlation analysis. Results Compared with the control group, the levels of total cholesterol (TC), TG, apolipoprotein (APO) B and VLDL in elderly and non-elderly group were significantly increased, while those in high-density lipoprotein HDL) had a decline, the P value of <0.05,0. 0 1,0 .0 5 or 0. Compared with the non-elderly group, TG in the non-elderly group was significantly higher than that in the elderly group (P <0.05), while HDL was lower in the elderly group than in the elderly group (P <0.05) There was a significant negative correlation between age and course of disease (γ = -0.22343, γ = -1.3443, P <0.001). Conclusions The lipid metabolism disorders are common in type 2 diabetic patients. However, the abnormal lipid metabolism in type 2 diabetic patients is more severe than that in type 2 diabetic patients, suggesting that more attention should be paid to the early prevention and treatment of lipid metabolism disorders in type 2 diabetic patients.
