目的探讨鲑鱼降钙素对骨质疏松性椎体骨折老年患者骨代谢、骨折愈合的促进作用。方法选取安徽医科大学附属巢湖医院2010年3月~2012年4月收治的老年骨质疏松性椎体骨折患者96例作为研究对象,随机分为观察组和对照组,各48例。2组患者均给予常规治疗,观察组患者在常规治疗基础上给予鲑鱼降钙素和钙剂治疗,对照组患者仅给予钙剂治疗,2组均连续治疗24周,观察骨折愈合情况及治疗前后骨代谢指标的变化情况。采用SPSS 17.0统计学软件进行数据处理,计数资料以百分率表示,采用χ2检验,计量资料以(xˉ±s)表示,采用t检验,P<0.05为差异有统计学意义。结果 X线检查显示观察组患者治疗4~6周骨折愈合率明显高于对照组(P<0.05);观察组患者治疗后骨钙素和骨密度明显升高,并高于对照组,差异有统计学意义(P<0.05);2组患者治疗前后血钙、血磷及血碱性磷酸酶差异均无统计学意义(P>0.05),但观察组术后血清骨碱性磷酸酶、尿钙/肌酐值均较治疗前下降,并低于同期对照组,差异有统计学意义(P<0.05)。结论鲑鱼降钙素治疗老年骨质疏松性椎体骨折患者能有效抑制骨吸收、促进骨形成,利于骨折愈合,效果良好,值得研究应用。
Objective To investigate the effects of salmon calcitonin on bone metabolism and fracture healing in elderly patients with osteoporotic vertebral fractures. Methods A total of 96 elderly patients with osteoporotic vertebral fractures admitted from March 2010 to April 2012 in Chaohu Hospital of Anhui Medical University were randomly divided into observation group (48 cases) and control group (48 cases). The patients in the two groups were given routine treatment. The patients in the observation group were treated with salmon calcitonin and calcium on the basis of routine treatment. The control group were treated with calcium only. The two groups were treated continuously for 24 weeks. The osteocalcin and bone mineral density in the observation group were significantly higher than those in the control group (P <0.05). There was no significant difference of serum Ca, P and ALP between the two groups before and after treatment (P> 0.05), but the serum levels of bone alkaline phosphatase, urine Calcium / creatinine values were lower than before treatment, and lower than the same period the control group, the difference was statistically significant (P <0.05). Conclusion Salmon calcitonin treatment of elderly patients with osteoporotic vertebral fractures can effectively inhibit bone resorption and promote bone formation, which is conducive to fracture healing, the effect is good, it is worth studying and applying.
