舌癌一般认为发生在舌前2/3部位的,比发生在舌根部的预后要好。因此对前者处理比较简单。作者回顾了126例舌前部鳞状细胞癌,每例随访至少达三年之久,发现其严重程度不亚于舌后部癌。舌根部癌发现一般均属晚期,可能是导致认为比舌前部癌更危及生命这一错误概念的原因。本文分析了舌癌治疗失败的原因,强调了临床上许多Ⅰ、Ⅱ期的癌(也即临床上颈淋巴结检查阴性者),事实上在病理检查时巳证明为Ⅲ期的了。由于这一结果,使得实际上已为Ⅲ期的癌,却给予Ⅰ、Ⅱ期的治疗。
Tongue cancer is generally thought to occur in the anterior 2/3 of the tongue and has a better prognosis than in the base of the tongue. Therefore, the former is relatively simple to handle. The authors reviewed 126 cases of anterior squamous cell carcinoma of the tongue. Each case was followed up for at least three years and found to be no less severe than that of the posterior tongue. Cancerous findings in the base of the tongue are generally advanced and may be the cause of the misconception that it is more life-threatening than tongue cancer. This article analyzes the reasons for the failure of tongue cancer treatment, and emphasizes many clinical stage I and II cancers (ie, clinically negative cervical lymph node examinations). In fact, during pathological examination, it was proved to be stage III. As a result of this result, in fact, it has been stage III cancer, but I, II treatment.
