目的探讨对老年股骨粗隆间骨折患者,采用抗旋股骨近端髓内钉(proximal femoral nail-rotation,PFNA)内固定手术,围手术期护理的方法及功能锻炼的重要性。方法对10例股骨粗隆间骨折实施PFNA内固定手术的患者,配合医师积极完善术前准备,术后加强生命体征观察及术前术后各种并发症的预防;依靠坚强内固定术后加强早期功能锻炼指导。结果本组病例通过手术治疗,术后积极有效的护理,手术效果满意,肢体功能恢复良好。术后无心脑血管、肺部、泌尿系感染发生,下肢胫后静脉血栓发生1例。结论PFNA内固定更符合生物力学的特点,手术创伤及出血少,固定牢固,能使病人尽早离床活动,早期加强功能锻炼指导,能减少并发症,恢复肢体功能,提高患者生活质量。 Objective To explore the importance of perioperative nursing and functional training in elderly patients with femoral intertrochanteric fracture treated with proximal femoral nail-rotation (PFNA). Methods Ten patients with femoral intertrochanteric fractures treated by PFNA internal fixation were instructed to cooperate with physicians to perfect the preoperative preparation, to observe the vital signs and to prevent various complications before and after operation. Early functional exercise guide. Results This group of patients by surgery, active and effective postoperative care, satisfactory surgical results, body function recovered well. No postoperative cardiovascular, pulmonary, urinary tract infection, lower extremity posterior tibial vein thrombosis occurred in 1 case. Conclusion PFNA is more biomechanical, less trauma, less bleeding and firm fixation. It can make patients get out of bed as early as possible and strengthen functional training early. It can reduce complications, restore limb function and improve patients’ quality of life.
