目的了解掌握青海三江源自然保护区范围内基层卫生医疗机构布鲁氏菌病(简称布病)防控能力现状。方法问卷调查三江源地区基层医疗卫生机构2010-2012年间布病防控的资源、执行能力及核心职责履行情况。结果疾病预防控制中心(简称CDC)系统:(1)资源:保护区范围内共有17个县(市)级CDC,专业人员总数257人,兼职布病防控的地方病科人员43人,检验人员27人。有17.7%的县为《布鲁氏菌病防治中央财政转移支付项目》县,有专项经费支持。(2)执行能力:各CDC均设有负责布病防控工作的地方病科和检验科,有41%实验室具备布病血清学检测能力。布病防控人员中本科以上学历占20.0%,高级职称占12.9%。(3)核心职能的履行:2010-2012年间仅有17.7%的县级CDC开展布病监测或干预项目。县级医院、乡镇卫生院系统:均未设置布病防控专职或兼职人员,检验科均未开展布病实验室检测项目。结论青海三江源自然保护区布病防控资源匮乏,人员经费与防控任务严重失衡,布病防控能力亟待提高。
Objective To understand the status quo of prevention and control of brucellosis (referred to as brucellosis) in primary health care institutions within the Sanjiangyuan Nature Reserve in Qinghai Province. Methods Questionnaire survey was carried out to investigate the implementation of resources and implementation capabilities and core responsibilities of brucellosis prevention and control in primary health care institutions in the Three River Source Regions during the period of 2010-2012. Results CDC system: (1) Resources: A total of 17 CDC counties (prefectures), 257 professionals and 43 local disease prevention and control bureaus were set up within the reserve. The inspectors 27 people. 17.7% of counties are “the central financial transfer prevention and control projects for brucellosis” counties with special funds. (2) Execution capacity: Each CDC has the endemic and laboratory departments responsible for the prevention and control of brucellosis, and 41% of the laboratories have brucellosis serological test capability. Cloth disease prevention and control personnel with bachelor degree or above accounted for 20.0%, senior titles accounted for 12.9%. (3) Fulfillment of core functions: Only 17.7% of county-level CDCs in 2010-2012 conducted brucellosis surveillance or intervention projects. County-level hospitals and township hospital systems: None of the full-time or part-time staff of brucellosis prevention and control departments were set up, and the brucellosis laboratory tests were not carried out by the laboratories. Conclusion There is a lack of resources for prevention and control of brucellosis in Sanjiangyuan Nature Reserve in Qinghai Province. There is a serious imbalance between staff funding and prevention and control tasks, and the prevention and control of brucellosis needs to be improved urgently.
