目的实验研究64层螺旋CT虚拟结肠镜360°虚拟解剖视图技术在测量直肠癌距肛门距离中的准确性。材料与方法洗净的新鲜猪结肠1段(约20cm),人工模拟直肠肿瘤样病变10枚。将模拟病变依次固定于猪结肠内,向模型内注入空气后浸入水箱中。采用GE Light Speed64层螺旋CT对模型进行扫描,将原始数据经0.625mm重组后传至AW4.2工作站,运用虚拟结肠镜的专用软件进行图像重组,在360°虚拟解剖视图中对肿瘤远端距起点之间的距离进行测量。每个病变的距离测量10次,取其平均值。将测量距离的平均值与病变真实距离进行比较,采用独立样本t检验行统计学处理。结果在360°虚拟解剖视图中,10枚模拟病变距测量起点间的距离均较准确地被测出,数值与实际距离均较接近,少数结果略大于实际距离。模拟病变距起点的实际距离为10~145mm,平均77.5mm;在360°虚拟解剖视图上测量的结果为9.6~142.4mm,平均76.5mm,两组数据间差异无统计学意义(t=0.049,P=0.961)。结论64层螺旋CT虚拟结肠镜360°虚拟解剖视图技术能较准确地测量直肠癌距肛门的距离,能够为外科直肠癌术式的选择提供重要依据。
Objective To study the accuracy of 360 ° virtual anatomical view of 64-slice spiral CT virtual colonoscopy in measuring the distance from rectum to the anus. Materials and methods Wash fresh pig colon 1 (about 20cm), simulated artificial rectal tumor-like lesions 10. Simulated lesions were fixed in the porcine colon, into the model after the infusion of air into the tank. The model was scanned with GE Light Speed 64-slice spiral CT. After the original data was reconstructed by 0.625mm, it was transmitted to AW4.2 workstation. The images were reconstructed using special software of virtual colonoscope. The distance between the start points is measured. The distance of each lesion is measured 10 times, whichever is the average. The mean value of the measured distance was compared with the true distance of the lesion, and the independent sample t-test was used for statistical analysis. Results In the 360 ° virtual anatomical view, the distance between the 10 simulated lesions and the starting point of the measurement was more accurately measured, the values were close to the actual distance, and a few results were slightly larger than the actual distance. The actual distance from the starting point of simulated lesions was 10 ~ 145mm with an average of 77.5mm. The measured results on the 360 ° virtual anatomical view were 9.6 ~ 142.4mm with an average of 76.5mm. There was no significant difference between the two groups (t = 0.049, P = 0.961). Conclusion The technique of 360 ° virtual colonography with 64-slice spiral CT can accurately measure the distance from the anus to the rectal cancer and provide an important basis for surgical rectal cancer surgery.
