目的:从分子水平探讨青春期鼻咽血管纤维瘤(juvenilenasopharyngealangiofibroma,JNA)与婴幼儿血管瘤的区别。方法:用免疫组织化学方法分别对25例JNA(JNA组),10例婴幼儿血管瘤(对照组Ⅰ),11例婴幼儿血管畸形(对照组Ⅱ)术后标本血管内皮细胞中血管内皮细胞增殖细胞核抗原(PCNA)、血管内皮生长因子(VEGF)、碱性成纤维细胞因子(bFGF)的表达进行检测。结果:JNA组血管内皮细胞中PCNA、VEGF表达呈阴性,血管瘤组血管内皮细胞中PCNA、VEGF表达阳性,对照组Ⅰ血管内皮细胞中PCNA、VEGF表达阴性,JNA组血管内皮细胞PCNA、VEGF表达与血管瘤组差异有统计学意义(P<0.01),与血管畸形组的差异无统计学意义(P>0.05);JNA组血管内皮细胞中bFGF表达阳性,对照组Ⅰ血管内皮细胞bFGF表达阳性,对照组Ⅱ血管内皮细胞bFGF表达阴性,JNA组血管内皮细胞中bFGF表达与对照组Ⅰ的差异无统计学意义(P>0.05),与对照组Ⅱ的差异有统计学意义(P<0.01)。结论:推测JNA的本质为先天性血管增殖畸形。
Objective: To explore the difference between juvenile nasopharyngeal angiofibroma (JNA) and infantile hemangiomas at the molecular level. Methods: Immunohistochemistry was used to detect the expression of vascular endothelial cells (VECs) in 25 cases of JNA group, 10 cases of infantile hemangiomas (control group Ⅰ) and 11 cases of infantile vascular malformations (control group Ⅱ) Proliferating cell nuclear antigen (PCNA), vascular endothelial growth factor (VEGF), basic fibroblast factor (bFGF) expression was detected. Results: The expression of PCNA and VEGF in JNA group was negative. The expression of PCNA and VEGF in vascular endothelial cells of JNA group was negative. The expression of PCNA and VEGF in VEC group Ⅰ was negative. The expression of PCNA and VEGF in JNA group (P <0.01), but there was no significant difference between them (P> 0.05). The expression of bFGF in vascular endothelial cells of JNA group was positive, while that in control groupⅠwas higher than that in vascular endothelial cells , While the expression of bFGF in control group Ⅱ was negative. The expression of bFGF in vascular endothelial cells of JNA group was not significantly different from that of control group Ⅰ (P> 0.05), but there was significant difference with that of control group Ⅱ (P0.01) . Conclusion: It is speculated that the nature of JNA is congenital vascular proliferation deformity.
