目的比较胃癌术后早期肠内营养(EEN)+肠外营养(PN)与术后早期完全胃肠外营养(TPN)对患者应激和免疫指标的影响。方法将择期进行胃癌根治手术患者随机分为EEN+PN组(22例)和TPN组(24例),两组患者分别于术后24h开始等热量、等氮营养治疗,比较两组患者术前与术后1周C-反应蛋白(CRP)、肿瘤坏死因子α(TNF-α)水平的差异。结果TPN组术后7d CRP显著高于EEN+PN组(P<0.05)。TPN组术后7d TNF-α组与EEN+PN组相比无明显增高(P>0.05)。结论EEN+PN在改善应激指标方面优于TPN,可成为消化道肿瘤患者术后首选的营养方式。
Objective To compare the effects of early postoperative enteral nutrition (EEN) + parenteral nutrition (PN) and postoperative early complete parenteral nutrition (TPN) on stress and immune parameters in patients with gastric cancer. Methods EEG patients (22 cases) and TPN group (24 cases) were randomly divided into EEN + PN group (24 cases). The patients in the two groups were treated with equal heat and nitrogen nutrition at 24 hours after operation, And the difference of C-reactive protein (CRP) and tumor necrosis factor alpha (TNF-alpha) level after 1 week. Results The CRP of TPN group was significantly higher than that of EEN + PN group on the 7th day (P <0.05). Compared with EEN + PN group, the level of TNF-α in TPN group was not significantly increased on the 7th day after operation (P> 0.05). Conclusion EEN + PN is superior to TPN in improving stress index, which may be the preferred nutritional method for postoperative patients with gastrointestinal cancer.
