目的:比较DLF和DCF方案治疗进展期胃癌的近期疗效。方法:65例初治晚期胃癌患者分为两组,DLF组予多西他赛45mg/m2静脉滴注,第1天;接以醛氢叶酸200mg/m2静脉滴注2h,注射后立即静脉推注5-FU400mg/m2,后续5-FU600mg/m2静脉持续输注22h,第1、2天给药;每2周重复1次,每2次为1个周期。DCF组予多西他赛75mg/m2第1天,顺铂75mg/m2第1天,5-FU750mg/(m2·d)连续5d。每3周重复1次为1个周期。结果:DLF组有效率35.5%,DCF组有效率38.2%,两组相比较无明显差异。DCF组Ⅲ～Ⅳ度白细胞减少的发生率达到了47.1%,明显高于DLF组的22.6%。DCF组的Ⅲ度呕吐发生率达11.8%,而DLF组无Ⅲ度以上呕吐发生。DLF组的Ⅲ度腹泻发生率为6.5%,而DCF组无Ⅲ度以上腹泻发生。结论:DLF和DCF方案治疗进展期胃癌疗效没有明显差异。在不良反应方面两组之间差异较大。
Objective: To compare the short-term effects of DLF and DCF regimens in the treatment of advanced gastric cancer. Methods: Sixty-five patients with newly diagnosed advanced gastric cancer were divided into two groups. The DLF group was given docetaxel 45mg / m2 intravenously on the first day. The patients were given intravenous infusion of dihydrofolate 200mg / m2 for 2 hours. Immediate intravenous injection Note 5-FU400mg / m2, follow-up 5-FU600mg / m2 intravenous infusion of 22h, the first and second days of administration; repeated once every two weeks, every two times for a cycle. DCF group to docetaxel 75mg / m2 on the first day, cisplatin 75mg / m2 on the first day, 5-FU750mg / (m2 · d) for 5d. Repeat once every 3 weeks for 1 cycle. Results: The effective rate was 35.5% in DLF group and 38.2% in DCF group, there was no significant difference between the two groups. The incidence of grade Ⅲ ~ Ⅳ leukopenia in DCF group was 47.1%, significantly higher than that in DLF group (22.6%). The incidence of grade Ⅲ vomiting in DCF group was 11.8%, while no grade Ⅲ or higher vomiting occurred in DLF group. The incidence of grade III diarrhea in the DLF group was 6.5%, while no grade III or higher diarrhea occurred in the DCF group. Conclusion: There is no significant difference between DLF and DCF in the treatment of advanced gastric cancer. Adverse reactions in the two groups vary greatly.
