目的:探讨支气管哮喘淋巴液和外周血IL-10、IL-12、T细胞亚群水平,及胸导管引流(TDD)的影响。方法:采用酶联免疫吸附试验(ELISA)检测31例中、重度哮喘病人TDD治疗0d、5d淋巴液和外周血的IL-10、IL-12及T细胞亚群。结果:哮喘淋巴液中IL-10明显高于正常对照组血浆水平,而IL-12则明显降低;TDD治疗5天,哮喘淋巴液和血浆中IL-10均明显下降,IL-12则显著升高达正常血浆水平。哮喘淋巴液中CD3、CD8百分率明显低于正常人外周血水平,而CD4/CD8比值、B细胞百分率则显著增高;哮喘淋巴液中CD3百分率、CD4/CD8比值显著高于其自身外周血水平;TDD治疗5天,哮喘淋巴液中CD4/CD8比值下降,同时外周血CD3、CD8百分率明显升高,CD4/CD8比值、B细胞百分率下降。结论:哮喘淋巴液中存在IL-10和IL-12产生或释放失调,以及T细胞亚群失衡。TDD可能通过引流部分细胞因子和T细胞,调整和促进细胞因子和T细胞亚群恢复正常。
Objective: To investigate the levels of IL-10, IL-12, T lymphocyte subsets in bronchial asthma and peripheral blood and the effect of thoracic ductal drainage (TDD). Methods: Serum levels of IL-10, IL-12 and T lymphocyte subsets in peripheral blood and lymphocytes were detected by enzyme linked immunosorbent assay (ELISA) in 31 cases of moderate and severe asthma patients. Results: The level of IL-10 in asthmatic lymph was significantly higher than that in normal control group, while the level of IL-12 was significantly decreased. On the 5th day of TDD treatment, the levels of IL-10 in lymph and plasma of asthma decreased significantly Up to normal plasma level. The percentage of CD3 and CD8 in asthmatic lymphatic fluid was significantly lower than that in normal people, while the ratio of CD4 / CD8 and the percentage of B cell were significantly increased. The percentage of CD3 and CD4 / CD8 in asthmatic lymph were significantly higher than those in peripheral blood. After 5 days of TDD treatment, the ratio of CD4 / CD8 in asthmatic lymph decreased. At the same time, the percentage of CD3 and CD8 in peripheral blood increased obviously, and the ratio of CD4 / CD8 and the percentage of B cell decreased. CONCLUSIONS: IL-10 and IL-12 production or release is imbalanced in asthmatic lymph and imbalance of T cell subsets. TDD may regulate and promote the normal return of cytokines and T cell subsets by draining some cytokines and T cells.
