目的探讨白细胞介素-5(IL-5)、C反应蛋白(CRP)水平在呼吸道感染患儿血浆中的表达及其临床意义。方法选取2012年7月至2014年6月成都天府新区人民医院收治的呼吸道感染患儿75例作为感染组,依据感染类型分为病毒组、支原体组、细菌组3个亚组,另选取同期体检中心的健康儿童30名作为对照组,所有受试者采用酶联免疫吸附法检测IL-5、CRP,采用ROC曲线分析二者对呼吸道细菌感染的诊断价值。结果感染组IL-5、CRP水平高于对照组,差异有统计学意义(P<0.05);在IL-5、CRP水平方面,细菌组>支原体组>病毒组,差异有统计学意义(P<0.05);ROC曲线分析结果显示,IL-5对小儿呼吸道细菌感染诊断特异度为63.28%,敏感度为72.59%,准确度为74.72%,CRP特异度为67.25%,敏感度为78.16%,准确度为81.26%,IL-5、CRP联合检测特异度为78.54%,敏感度为84.29%,准确度为93.19%。结论IL-5、CRP均可作为小儿呼吸道细菌感染的重要诊断指标,两者联合检测具有更高的特异度、敏感度、准确度。
Objective To investigate the expression of interleukin-5 (IL-5) and C-reactive protein (CRP) in plasma of children with respiratory tract infection and its clinical significance. Methods From July 2012 to June 2014, 75 cases of children with respiratory tract infection admitted to Tianfu New District People’s Hospital of Chengdu were selected as infection group. According to the infection types, 75 cases were divided into three subgroups: viral group, mycoplasma group and bacterial group. Thirty healthy children in the center were used as the control group. All subjects were tested for IL-5 and CRP by enzyme-linked immunosorbent assay (ELISA), and the diagnostic value of the two methods for the diagnosis of respiratory tract bacterial infection by ROC curve analysis. Results The levels of IL-5 and CRP in the infected group were significantly higher than those in the control group (P <0.05). The levels of IL-5 and CRP in the bacterial group, the mycoplasma group and the virus group were significantly different (P <0.05). The ROC curve analysis showed that the specificity of IL-5 in diagnosis of respiratory tract bacterial infection in children was 63.28%, the sensitivity was 72.59%, the accuracy was 74.72%, the specificity of CRP was 67.25% and the sensitivity was 78.16% The accuracy was 81.26%. The specificity of combined detection of IL-5 and CRP was 78.54%, the sensitivity was 84.29% and the accuracy was 93.19%. Conclusion Both IL-5 and CRP can be used as important diagnostic indicators of respiratory tract bacterial infections in children. The combined detection of IL-5 and CRP has higher specificity, sensitivity and accuracy.
