多层螺旋CT动态增强扫描对孤立性肺结节的诊断价值

目的:分析多层螺旋计算机断层扫描(CT)动态增强扫描对孤立性肺结节的诊断价值。方法:选取东莞市沙田医院2010年至2014年收治的孤立性肺结节患者150例,所有患者均给予多层螺旋CT动态增强扫描,且影像学资料完整。分析患者诊断结果,并与穿刺活检结果或手术病理诊断结果进行对照,比较良恶性病变的诊断灵敏度、特异性。记录良性结节、炎性结节、恶性结节患者在动态增强扫描的不同时间CT值的变化情况,同时计算并比较其各自的强化峰值以及孤立性肺结节强化峰值、主动脉强化值的比值。结果:经多层螺旋CT动态增强扫描,共检出良性病变患者66例,恶性病变患者84例,检出率分别为44.0%和56.0%,其中恶性病变的诊断灵敏度、特异性分别为89.86%、95.06%。不同类型结节患者的PH值、SPH/PPH计算结果比较有炎性结节>恶性结节>良性结节的规律,差异具有统计学意义(P<0.05)。结论:多层螺旋CT动态增强扫描应用于孤立性肺结节的诊断中,不仅有扫描速度快、分辨率高的特点,而且后处理技术功能强,通过CT值的变化规律,可以对病变的良恶性进行有效鉴别。 Objective: To analyze the diagnostic value of dynamic contrast-enhanced multislice computed tomography (CT) in the diagnosis of solitary pulmonary nodules. Methods: 150 cases of solitary pulmonary nodules were selected from Shatin Hospital of Dongguan City from 2010 to 2014. All the patients were given multi-slice spiral CT dynamic contrast-enhanced scanning and the imaging data were complete. Analysis of patient diagnostic results, and biopsy results or surgical pathological diagnosis compared to compare the diagnostic sensitivity and specificity of benign and malignant lesions. The changes of CT value of dynamic nodules and malignant nodules in patients with benign nodules, inflammatory nodules and malignant nodules were recorded, and their respective enhancement peaks and their solitary pulmonary nodule enhancement peak values ​​and aorta enhancement values ratio. Results: 66 cases of benign lesions and 84 cases of malignant lesions were detected by dynamic enhanced multi-slice spiral CT. The detection rates were 44.0% and 56.0% respectively. The diagnostic sensitivity and specificity of malignant lesions were 89.86% , 95.06%. The results of SPH / PPH in different types of patients with nodules showed the regularity of inflammatory nodules> malignant nodules> benign nodules, with statistical significance (P <0.05). Conclusion: Multi-slice spiral CT dynamic enhanced scan in the diagnosis of solitary pulmonary nodules not only has the characteristics of fast scanning speed and high resolution, but also possesses the powerful post-processing technique.
