目的探讨P14ARF、P16INK4a的基因表达与高危型人乳头瘤病毒DNA(HPV DNA)感染的关系。方法采用免疫组化EnVision法,对130例宫颈鳞癌、宫颈上皮内瘤变60例、20例正常宫颈组织进行P16INK4a、P14ARF蛋白基因检测,并采用原位杂交法检测130例宫颈鳞癌、宫颈上皮内瘤变60例、20例正常宫颈组织中的HPV DNA。结果宫颈鳞癌组中P16INK4a、P14ARF蛋白表达率分别为100.0%(130/130)和96.9%(126/130);在60例宫颈上皮内瘤变组中其表达率分别为85.0%(51/60)、81.7%(49/60);在20例正常宫颈组中其表达率分别为10.0%(2/20)、15.0%(3/20),宫颈鳞癌组显著高于正常对照组(P<0.01);正常宫颈组与宫颈上皮内瘤变及宫颈鳞癌组P16INK4a和P14ARF的表达呈显著差异(P<0.05)。HPV16/18在130例宫颈鳞癌中86例(66.2%)表达阳性;在宫颈上皮内瘤变中39例(65.0%)表达阳性;P16INK4a、P14ARF阳性率HPV DNA阳性宫颈鳞癌、宫颈上皮内瘤变组与HPV DNA阴性宫颈鳞癌、宫颈上皮内瘤变组比较无显著差异(P>0.05)。结论P16INK4a、P14ARF蛋白在宫颈上皮内瘤变及宫颈鳞癌组织中呈高表达,有较高的特异性和敏感性,可作为宫颈癌前病变及宫颈癌的诊断指标。
Objective To investigate the relationship between the gene expression of P14ARF and P16INK4a and the infection of human papillomavirus DNA (HPV DNA). Methods Immunohistochemical EnVision method was used to detect the protein of P16INK4a and P14ARF in 130 cases of cervical squamous cell carcinoma, 60 cases of cervical intraepithelial neoplasia and 20 cases of normal cervical tissue. In situ hybridization was used to detect 130 cases of cervical squamous cell carcinoma, Intraepithelial neoplasia in 60 cases, 20 cases of normal cervical tissue HPV DNA. Results The positive rates of P16INK4a and P14ARF in cervical squamous cell carcinoma were 100.0% (130/130) and 96.9% (126/130), respectively. The positive rate of P16INK4a and P14ARF in cervical squamous cell carcinoma was 85.0% (51 / 60), 81.7% (49/60) respectively. The expression rates in cervical squamous cell carcinoma were significantly higher than those in normal control group (10.0% (2/20), 15.0% (3/20) P <0.01). The expression of P16INK4a and P14ARF in normal cervix, cervical intraepithelial neoplasia and cervical squamous cell carcinoma were significantly different (P <0.05). HPV16 / 18 was positive in 86 cases (66.2%) of 130 cases of cervical squamous cell carcinoma and 39 cases (65.0%) in cervical intraepithelial neoplasia. HPV16 / 18 positive rate of HPV DNA positive cervical squamous cell carcinoma, cervical intraepithelial neoplasia There was no significant difference between the tumorigenic group and HPV DNA negative cervical squamous cell carcinoma and cervical intraepithelial neoplasia group (P> 0.05). Conclusion P16INK4a and P14ARF proteins are highly expressed in cervical intraepithelial neoplasia and cervical squamous cell carcinoma, which have high specificity and sensitivity, which can be used as a diagnostic index of cervical precancerous lesions and cervical cancer.
