目的重新理解潜在HCO3-概念,正确应用潜在HCO3-公式。方法利用人体血浆中阴离子之间的特殊变化关系及高阴离子间隙(AG)代谢性酸中毒的特征进行研究,阐述潜在HCO3-概念新观点,并推导新的潜在HCO3-公式。结果过去将本该用于高AG代谢性酸中毒判断的潜在HCO3-公式误用在代谢性碱中毒的补充判断上,结果导致高AG代谢性酸中毒的判断少用了一个判断条件而代谢性碱中毒又使用了不成立的潜在HCO3-公式进行补充判断而出现高AG代谢性酸中毒和代谢性碱中毒的双重过度判断。结论原潜在HCO3-概念理解有误,不符合逻辑,其潜在HCO3-公式设计有问题,本文推导的潜在HCO3-公式经检验符合逻辑,而且是用在高AG代谢性酸中毒判断上。这样就可以避免高AG代谢性酸中毒与代谢性碱中毒的双重误判。
Objective To re-understand the concept of potential HCO3- and to apply the potential HCO3-formula correctly. Methods The special relationship between the anions in human plasma and the characteristics of metabolic acidosis of high anion gap (AG) were studied. The new concept of potential HCO3 - was expounded and a new potential HCO3 - formula was derived. Results In the past, the potential HCO3-formula, which should be used for the judgment of high AG metabolic acidosis, should be misused in the supplementary judgment of metabolic alkalosis, resulting in less judgmental use of high metabolic metabolic acidosis Alkalosis and the use of potential HCO3-formula does not exist to determine the additional judgment of high AG metabolic acidosis and metabolic alkalosis double overestimation. CONCLUSIONS The original concept of HCO3-was incorrect and not logical, and its potential HCO3-formula design was problematic. The potential HCO3-formula derived in this paper was logically validated and used in the judgment of high AG metabolic acidosis. This avoids the dual misjudgment of high AG metabolic acidosis and metabolic alkalosis.
