目的:应用二维斑点追踪技术评价2型糖尿病(T2DM)患者早期左心房功能。方法:搜集T2DM患者56例,把其中27例无微血管病者作为A组,29例合并微血管病者作为B组,30例正常健康志愿者作为对照组(C组)。采集静息状态下心尖两腔切面和心尖四腔切面二维动态图像,应用二维斑点追踪技术(2D-STE)技术获取左心房各壁(房间隔、侧壁、下壁及前壁)在左心室收缩期、舒张早期和舒张晚期的峰值应变率及左心房整体应变(SRs、SRe、SRa、Gs),并计算其平均值(mSRs、mSRe、mSRa)。应用实时三维超声技术测得左心房各容积参数、左心房被动射血分数(LAPEF)、左心房主动射血分数(LAAEF)及左房扩张指数(LAEI)。另外,各组均行常规超声心动图检查。结果:与C组比较,A组和B组左心房Gs、mSRs、mSRe降低,mSRa升高(均P<0.01)。A和B组mSRa与LAAEF呈显著正相关(r=0.785,0.813,均P<0.01)。结论:2D-STE可较好地反映T2DM患者早期左心房功能。
Objective: To evaluate early left atrial function in patients with type 2 diabetes mellitus (T2DM) by two-dimensional speckle tracking. Methods: Totally 56 patients with T2DM were enrolled in this study. Twenty-seven patients without microvascular disease were selected as group A, 29 patients with microvascular disease as group B and 30 healthy volunteers as control group (group C). Two-dimensional dynamic images of the apex and quadruple-lumen of the left atrium were collected by 2D-STE technique. The atrial septum, lateral wall, inferior wall and anterior wall of the left atrium Left ventricular systole, early diastolic and late diastolic peak strain rate and left atrial total strain (SRs, SRe, SRa, Gs), and calculate the average (mSRs, mSRe, mSRa). The volumetric parameters of left atrium, LAPEF, LAAEF and LAEI were measured by real-time three-dimensional ultrasound. In addition, each group underwent routine echocardiography. Results: Compared with group C, the left atrium Gs, mSRs and mSRe decreased and mSRa increased in group A and group B (all P <0.01). There was a significant positive correlation between mSRa and LAAEF in groups A and B (r = 0.785,0.813, all P <0.01). Conclusion: 2D-STE can better reflect the early left atrial function in patients with T2DM.
