目的:探讨瞬态声诱发耳声发射(TEOAE)在新生儿听力筛查中的应用及听力损伤与疾病的关系。方法:将正常足月新生儿997例和同期新生儿重症监护病房(NICU)的新生儿241例分别使用手持式耳声发射仪进行听力跟踪筛查。结果:正常新生儿组TEOAE检查总通过率99.20%,8例未通过者经听性脑干反应(ABR)检查,1例(0.10%)确定为不正常;NICU组TEOAE检查总通过率92.53%,18例未通过者经ABR检查,10例(4.15%)确定为不正常。两组新生儿听力损伤发生率分别为0.10%和4.15%,比较差异有统计学意义(P<0.05)。结论:TEOAE应用于新生儿听力筛查有客观、无创、快捷、方便等优点,对新生儿听力筛查准确性和可靠性的提高有重要意义。
Objective: To investigate the application of transient acoustic evoked otoacoustic emissions (TEOAE) in neonatal hearing screening and the relationship between hearing loss and disease. Methods: One hundred and ninety-seven newborn infants with normal full-term neonates and 241 neonates with neonatal intensive care unit (NICU) underwent the follow-up screening with hand-held otoacoustic transmitters. Results: The total passing rate of TEOAE in normal neonates was 99.20%. Eight patients who failed to pass the auditory brainstem response (ABR) and one patient (0.10%) were abnormal. The total passing rate of TEOAE in NICU was 92.53% , 18 cases of failed by ABR examination, 10 cases (4.15%) identified as abnormal. The incidence of neonatal hearing loss in the two groups were 0.10% and 4.15%, respectively, with statistical significance (P <0.05). Conclusion: The application of TEOAE in neonatal hearing screening is objective, noninvasive, quick and convenient. It is of great significance to improve the accuracy and reliability of neonatal hearing screening.
