目的 :研究低分子量肝素 (low- molecular- weight heparin,L MWH)对人工晶体植入术后眼内纤维蛋白渗出的预防作用。方法 :110例老年性白内障随机分为 2组 ,并盲法给药。在进行白内障囊外摘除及后房人工晶体植入手术中 ,对照组灌注液为乳酸钠林格氏液 ,L MWH组于灌注液中加入 L MWH—速避凝 (fraxiparine) ,浓度为 6 u/ ml,。术中观察眼内出血 ,术后 1、 3及 7天观察眼内纤维蛋白渗出、房水混浊及眼内出血情况。结果 :L MWH组术后第 1、 3、 7天眼内纤维蛋白渗出及房水混浊轻于对照组(P<0 .0 5 ) ,两组术中及术后出血无显著性差异 (P>0 .1)。结论 :L MWH能有效抑制人工晶体植入术后眼内纤维蛋白渗出反应 ,并且不增加眼内出血危险性。 Objective: To study the preventive effect of low molecular weight heparin (L MWH) on intraocular fibrin exudation after intraocular lens implantation. Results: The intraocular fibrin exudation and aqueous humor of the L MWH group were lighter than those of the control group (P <0.05) on the 1st, 3rd, and 7th day postoperatively (P <0.05), and there was no significant difference between the two groups in intraoperative and postoperative bleeding (P > 0 .1). CONCLUSION: L MWH can effectively inhibit intraocular fibrin exudation after intraocular lens implantation without increasing the risk of intraocular hemorrhage.
