低分子量肝素预防人工晶体植入术后眼内纤维蛋白渗出的临床研究

目的 :研究低分子量肝素 (low- molecular- weight heparin,L MWH)对人工晶体植入术后眼内纤维蛋白渗出的预防作用。方法 :110例老年性白内障随机分为 2组 ,并盲法给药。在进行白内障囊外摘除及后房人工晶体植入手术中 ,对照组灌注液为乳酸钠林格氏液 ,L MWH组于灌注液中加入 L MWH—速避凝 (fraxiparine) ,浓度为 6 u/ ml,。术中观察眼内出血 ,术后 1、 3及 7天观察眼内纤维蛋白渗出、房水混浊及眼内出血情况。结果 :L MWH组术后第 1、 3、 7天眼内纤维蛋白渗出及房水混浊轻于对照组(P<0 .0 5 ) ,两组术中及术后出血无显著性差异 (P>0 .1)。结论 :L MWH能有效抑制人工晶体植入术后眼内纤维蛋白渗出反应 ,并且不增加眼内出血危险性。 Objective: To study the preventive effect of low molecular weight heparin (L MWH) on intraocular fibrin exudation after intraocular lens implantation. Methods: One hundred and ten cases of senile cataract were randomly divided into two groups and given blindly. In extracapsular cataract extraction and posterior chamber intraocular lens implantation surgery, the control group perfusion solution of sodium lactate Ringer’s solution, L MWH group in the perfusion solution was added L MWH fraxiparine, the concentration of 6 u / ml Intraoperative bleeding was observed within 1, 3 and 7 days after intraocular fibrin exudate, aqueous humor and intraocular hemorrhage. Results: The intraocular fibrin exudation and aqueous humor of the L MWH group were lighter than those of the control group (P <0.05) on the 1st, 3rd, and 7th day postoperatively (P <0.05), and there was no significant difference between the two groups in intraoperative and postoperative bleeding (P > 0 .1). CONCLUSION: L MWH can effectively inhibit intraocular fibrin exudation after intraocular lens implantation without increasing the risk of intraocular hemorrhage.
