目的:探讨亚低温治疗对心跳骤停复苏成功后并发癫痫患者预后的影响。方法:回顾性分析我院79例于2013-12-2015-12进行心脏骤停复苏成功后并发癫痫患者。将患者分为实验组和对照组,其中实验组39例,对照组40例,选取实验组患者均进行亚低温治疗,对照组患者进行常规治疗,并在治疗过程中使用脑电图监测。记录患者病历中本研究所需数据,分析使用亚低温治疗对患者预后的影响。结果:在治疗过程中,共有40例患者发生了癫痫,实验组发生癫痫的患者有14例,对照组发生癫痫的患者有26例,对照组的癫痫发生率显著高于实验组(χ~2=6.69,P<0.05)。实验组和对照组患者在心肺复苏后和治疗中的癫痫发生率差异均无统计学意义,在治疗后癫痫的发生率差异有统计学意义(χ~2=5.23,P<0.05);进行治疗的患者58例死亡,其中并发癫痫患者40例全部死亡;院内治疗过程中死亡的患者有42例,其中有并发癫痫患者27例;出院后在心跳骤停28d内死亡的患者有16例,其中有并发癫痫患者13例。未发癫痫患者在心跳骤停28d内死亡的有18例,病死率为46%,并发癫痫患者病死率为100%,显著高于未发癫痫患者(χ~2=29.34,P<0.05)。结论:亚低温治疗能降低心跳骤停复苏成功后患者的癫痫发生率,但对心跳骤停复苏成功后并发癫痫患者的预后情况较差。
Objective: To investigate the effect of mild hypothermia on the prognosis of patients with epilepsy after success of cardiac arrest and resuscitation. Methods: A retrospective analysis of 79 cases of hospitalized patients with epilepsy after successful resuscitation from cardiac arrest in 2013-12-2015-12. The patients were divided into experimental group and control group, of which 39 cases in the experimental group and 40 cases in the control group. The patients in the experimental group were all treated with mild hypothermia. The control group received routine treatment and monitored by EEG during the treatment. The data required for this study in the patient’s medical records were recorded and the effect of mild hypothermia on the prognosis of the patients was analyzed. Results: A total of 40 patients developed epilepsy during the course of treatment. There were 14 epilepsy patients in the experimental group and 26 patients in the control group with epilepsy. The incidence of epilepsy in the control group was significantly higher than that in the experimental group (χ ~ 2 = 6.69, P <0.05). There was no significant difference in the incidence of epilepsy between the experimental group and the control group after cardiopulmonary resuscitation (CPR) and after treatment (P <0.05), and there was a significant difference in the incidence of epilepsy after treatment (χ ~ 2 = 5.23, Of the 58 patients died, of which 40 cases were complicated with epilepsy all died; 42 patients died of intra-hospital treatment, including 27 cases of complicated epilepsy; discharge in 28 days after cardiac arrest in patients with cardiac arrest in 16 cases, of which There are 13 cases of patients with epilepsy. Epilepsy patients died in 28 days after cardiac arrest, 18 cases, the mortality rate was 46%, the mortality of patients with epilepsy was 100%, significantly higher than those without epilepsy (χ ~ 2 = 29.34, P <0.05). Conclusion: Mild hypothermia can reduce the incidence of epilepsy in patients with successful cardioversion, but the prognosis of patients with epilepsy after successful cardioversion is poor.
