目的:观察平喘汤联合哮喘全球防治创议(GINA)阶梯方案对肺脾两虚支气管哮喘缓解期患儿血清超敏C反应蛋白(hs-CRP)、免疫球蛋白E(lg E)、肺功能、哮喘控制及生活质量的影响。方法:将110例肺脾两虚型支气管哮喘缓解期患儿随机分为观察组和对照组各55例。对照组采用GINA阶梯方案治疗,观察组在对照组用药基础上给予平喘汤治疗,2组均用药3月。观察2组治疗前后血清hs-CRP、lg E水平及肺功能、生活质量的变化,治疗后比较2组哮喘的控制情况。结果:治疗后,2组血清hs-CRP及lg E水平均较治疗前降低(P<0.05),FEV_1、FEV_1/FVC值均较治疗前升高(P<0.05);观察组的血清hs-CRP及lg E水平均低于对照组(P<0.05),FEV_1、FEV_1/FVC值均高于对照组(P<0.05)。治疗后观察组哮喘总控制率90.9%,对照组哮喘总控制率74.5%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组症状维度评分、活动受限维度评分、情感维度评分及总评分均较治疗前升高(P<0.05);观察组的各项评分均高于对照组(P<0.05)。结论:平喘汤联合GINA阶梯方案治疗肺脾两虚型支气管哮喘缓解期,能有效降低患儿的血清hs-CRP及lg E水平,改善肺功能,提高哮喘控制率和生活质量。
OBJECTIVE: To observe the effect of Pingchuan decoction combined with Asthma GINA ladder on serum hs-CRP, IgE, pulmonary function in children with bronchial asthma with remission of lung and spleen deficiency, , Asthma control and quality of life. Methods: A total of 110 children with bronchial asthma of lung and spleen deficiency type were randomly divided into observation group and control group with 55 cases each. The control group was treated with the GINA ladder protocol. The observation group was treated with Pingchuan Decoction on the basis of the control group, and both groups were given drugs for 3 months. The changes of serum hs-CRP, lgE, pulmonary function and quality of life before and after treatment were observed. The control of asthma in two groups were compared after treatment. Results: After treatment, the levels of hs-CRP and lgE in both groups were significantly lower than those before treatment (P <0.05), FEV_1 and FEV_1 / FVC were higher than those before treatment (P <0.05) CRP and lg E levels were lower than the control group (P <0.05), FEV_1, FEV_1 / FVC values were higher than the control group (P <0.05). After treatment, the total control rate of asthma in observation group was 90.9%, while that of control group was 74.5%. There was significant difference between the two groups (P <0.05). After treatment, the scores of symptom dimensions, limited activity dimensions, emotional dimension scores and total scores of two groups were significantly higher than those before treatment (P <0.05). The score of the observation group was higher than that of the control group (P <0.05). Conclusion: Pingchuan Decoction combined with GINA ladder regimen in the treatment of bronchial asthma remission of lung and spleen deficiency can effectively reduce the serum hs-CRP and IgE in children, improve lung function and improve the control rate of asthma and quality of life.
