目的 :评价保留髁突的颞下颌关节外侧成形术在生长期患者髁突的改建效果。方法 :对2010年1月—2014年12月采用外侧成形术治疗的单侧颞下颌关节强直生长期患者进行CT分析,采用Pro Plan CMF 1.4软件对术后1周和随访1年以上的健、患侧髁突-下颌支高度和颏点偏斜量进行测量。采用SPSS 17.0软件包对数据进行配对t检验。结果:9例患者纳入研究,年龄5~15岁,平均10.2岁。术后随访1~3 a,平均1.94 a。健侧髁突-下颌支平均生长2.34 mm,患侧2.70 mm,两者无统计学差异(P=0.31)。所有患者的髁突都有不同程度的直立改建。9例患者术前颏点偏斜平均3.69 mm,平均随访1.94 a,偏斜2.92 mm,两者无统计学差异(P=0.16)。结论:颞下颌关节强直的外侧成形术能够保留髁突的生长潜能,防止颌骨畸形进一步加重。
OBJECTIVE: To evaluate the effect of condylar remodeling in patients with condyles undergoing temporomandibular arthroplasty. Methods: CT analysis of unilateral temporomandibular joint antrograde patients from January 2010 to December 2014 was carried out by means of Pro Plan CMF 1.4 software. The results of one week follow-up and more than one year of follow-up were analyzed. Ipsilateral condyle - mandibular branch height and chin point deviation measured. Data was paired t-test using SPSS 17.0 software package. Results: Nine patients were enrolled in the study, aged 5-15 years, mean 10.2 years. The patients were followed up for 1-3 years, with an average of 1.94 a. Contralateral condyle - mandibular branch of the average growth of 2.34 mm, ipsilateral 2.70 mm, no significant difference between the two (P = 0.31). All patients had condyles with varying degrees of vertical remodeling. The average preoperative genioglossis deviation was 3.69 mm in 9 patients, with an average follow-up of 1.94 a and a skew of 2.92 mm. There was no significant difference between the two groups (P = 0.16). CONCLUSIONS: Torso-temporomandibular joint arthroplasty can preserve the condyle’s growth potential and prevent further deformity of the jaw.
