目的了解心脏起搏术后患者生理活动量与血浆心房利钠肽(ANP)、脑钠肽(BNP)水平的相关性及不同等级生理活动量之间血浆ANP、BNP水平的差异。方法用放射免疫法测定62例起搏患者的血浆ANP、BNP浓度,其中DDD17例,VVI39例,VVIR2例,VDD4例,根据特定活动量等级问卷调查来判断起搏患者的生理活动量,选择Ⅰ级、Ⅱ级及Ⅲ级入组进行研究。结果起搏患者生理活动量与血浆ANP水平有良好的相关性(r=0.39,P<0.01),生理活动量与血浆BNP水平相关性更显著(r=0.63,P<0.01)。血浆ANP水平在生理活动量Ⅲ级>Ⅱ级(P<0.05),>Ⅰ级(P<0.01),血浆BNP水平在生理活动量Ⅲ级>Ⅱ级(P<0.001),>Ⅰ级(P<0.001)。结论起搏患者的生理活动量与血浆ANP、BNP水平显著相关,可以用血浆ANP、BNP水平大致判断起搏患者的生理活动量,间接判断患者的心功能。且以血浆BNP水平衡量患者的生理活动量较血浆ANP水平更敏感。
Objective To investigate the relationship between the activity of plasma atrial natriuretic peptide (ANP) and brain natriuretic peptide (BNP) in patients after cardioversion and the differences of plasma ANP and BNP levels among different levels of physical activity. Methods The plasma ANP and BNP concentrations were measured by radioimmunoassay in 62 patients with pacing. Among them, 17 cases of DDD, 39 cases of VVI, 2 cases of VVIR and 4 cases of VDD were measured. The physical activity of pacing patients was determined according to the questionnaire survey of specific activity levels. Grade, grade Ⅱ and Ⅲ into the study group. Results There was a good correlation between the amount of physical activity and plasma ANP level in patients with pacing (r = 0.39, P <0.01). The correlation between physical activity and plasma BNP level was more significant (r = 0.63, P <0.01). The levels of plasma ANP were significantly higher than those of normal group (P <0.05), level Ⅰ (P <0.05), level of plasma BNP <0.001). Conclusion The amount of physiological activity in patients with pacing is significantly correlated with plasma ANP and BNP levels. The level of plasma ANP and BNP can be used to determine the physiological activity of patients with pacing and determine the cardiac function indirectly. And the level of plasma BNP measured in patients with physiological activity than plasma ANP levels are more sensitive.
