目的:分析门诊药房患者退药原因及其对策。方法:选取2012年10月1日—2013年3月31日期间门诊退药处方,分析其退药原因并提出其解决对策。结果:经统计,2012年10月1日—2013年3月31日门诊退药处方230张,患者退药的主要原因为药物不良反应、拒绝用药、治疗方案的调整或出院转院、药品重复、药房缺药等等。结论:制订退药管理制度,设置退药流程,建立“一切为病人为中心”的服务理念,加强医务人员的行为规范,定期分析退药原因。
Objective: To analyze the reasons and countermeasures of withdrawal from patients in outpatient pharmacy. Methods: The outpatient prescriptions for prescription withdrawal from October 1, 2012 to March 31, 2013 were selected to analyze the reasons for withdrawal and the countermeasures were given. Results: According to the statistics, from October 1, 2012 to March 31, 2013, outpatient retreat prescription was 230. The main reasons for patient withdrawal were adverse drug reactions, refusal of medication, adjustment of treatment regimen or discharge from hospital, drug duplication, Drug shortage and so on. Conclusion: To establish a withdrawal management system, set up a withdrawal process, establish a service philosophy of “all for the patient as the center”, strengthen the code of conduct of medical staff, regular analysis of the reasons for withdrawal.
