[目的]探讨胃癌患者腹腔冲洗液中CEAmRNA、DDCmRNA及PSPHmRNA表达情况与腹膜微转移关系。[方法]采用RT-PCR技术检测80例胃癌患者及16例胃良性肿瘤患者的腹腔冲洗液中的CEAmRNA、DDCmRNA及PSPHmRNA表达水平,并与传统的腹腔冲洗细胞学(PLC)对比分析,寻找与胃癌腹膜微转移有关的肿瘤标志物。[结果]胃癌组分别与胃良性瘤组患者比较,腹腔冲洗液中CEAmRNA、DDCmRNA、PSPHmRNA表达阳性率明显增高(P﹤0.05),其中CEAmRNA最高,其次为DDCmRNA,PSPHmRNA的阳性率较低(P﹤0.05);CEAmRNA、DDCmRNA、PSPHmRNA这3种指标在临床分期、组织学类型、pTNM、淋巴结转移及肝转移间阳性率比较,统计分析显示均有统计学意义(P﹤0.05),而在肿瘤部位方面,差异无统计学意义,随着临床分期的增加,这些肿瘤标志物的表达率也明显升高,有淋巴结及远处转移的患者较未转移的患者阳性率也明显升高;随着肿瘤浸润深度的增加,CEAmRNA、DDCmR-NA、PSPHmRNA及PLC阳性率也明显增加,统计分析显示差异有统计学意义(P﹤0.01)。[结论]采用RT-PCR技术检测胃癌患者腹腔冲洗液中的CEAmRNA及DDCmRNA表达,可以用于预测胃癌患者腹膜微转移。
[Objective] To investigate the relationship between CEAmRNA, DDCmRNA and PSPH mRNA expression and peritoneal micrometastasis in peritoneal washings of patients with gastric cancer. [Methods] The expression of CEAmRNA, DDCmRNA and PSPHmRNA in peritoneal washings of 80 gastric cancer patients and 16 gastric benign tumor patients were detected by RT-PCR, and compared with traditional peritoneal washing cytology (PLC) Gastric cancer peritoneal micrometastases related tumor markers. [Result] The positive rates of CEAmRNA, DDCmRNA and PSPHmRNA in peritoneal washings were significantly higher than those in gastric benign tumor patients (P <0.05), of which CEAmRNA was the highest, followed by DDCmRNA, and the positive rate of PSPHmRNA was lower (P <0.05). The positive rates of CEAmRNA, DDCmRNA and PSPHmRNA in clinical stage, histological type, pTNM, lymph node metastasis and liver metastasis were statistically significant (P <0.05) The difference was not statistically significant. With the increase of clinical stage, the expression rate of these tumor markers was also significantly increased, and the positive rate of patients with lymph node and distant metastasis than those without metastasis was also significantly increased. With the The tumor invasion depth increased, CEAmRNA, DDCmR-NA, PSPHmRNA and PLC positive rate also increased significantly, statistical analysis showed that the difference was statistically significant (P <0.01). [Conclusion] The detection of CEAmRNA and DDCmRNA in peritoneal washings of gastric cancer patients by RT-PCR can be used to predict peritoneal micrometastasis in patients with gastric cancer.
