目的探讨2型糖尿病(T2DM)并周围神经病变(DPN)患者的肝功能变化。方法分别选择2015年1月—2016年1月在安徽医科大学附属安庆医院接受治疗的2型糖尿病合并DPN(DPN组)和未合并DPN(非DPN组)患者各100例作为观察组,选择同期来我院健康体检患者200例作为对照组,采用生化检验法分别检测血清中丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)和γ谷氨酰转移酶(γ~(-GT))的含量变化。结果 DPN组及非DPN组中肝功能异常(ALT和γ~(-GT)浓度升高)的检出率均较对照组显著升高,与非DPN组相比,高水平的ALT和γ~(-GT)在DPN组中发生率更高,而另一个指标(AST浓度升高)出现异常的例数在3组中差异无统计学意义;非DPN组及DPN组患者血清中ALT和γ~(-GT)较对照组水平升高(P<0.05),DPN组较非DPN组患者血清中ALT和γ~(-GT)也明显升高(P<0.05),且DPN组患者随着病情加重,ALT和γ~(-GT)水平呈升高趋势(P<0.05);DPN发病各危险因素多元Logistic回归分析结果显示,ALT和γ~(-GT)是2型糖尿病合并DPN患者的危险因素。结论T2DM,DPN与肝功能变化有关,监测并控制ALT和γ~(-GT)水平有利于对DPN进行有效干预并改善其预后。
Objective To investigate the changes of liver function in patients with type 2 diabetes mellitus (T2DM) and peripheral neuropathy (DPN). Methods 100 cases of type 2 diabetes mellitus with DPN (DPN group) and non-DPN group (non-DPN group) were treated in Anqing Hospital of Anhui Medical University from January 2015 to January 2016 respectively as the observation group, A total of 200 healthy volunteers from our hospital were enrolled in this study. Serum levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST) and γ-glutamyl transferase (γ-GT) ) Content changes. Results The detection rates of abnormal liver function (ALT and γ-Gt concentration) in DPN group and non-DPN group were significantly higher than those in control group. Compared with non-DPN group, high ALT and γ- (-GT) were higher in DPN group, but there was no significant difference in the other index (AST concentration) among the three groups. The levels of ALT and γ in serum of non-DPN group and DPN group (P <0.05). The levels of ALT and γ-GT in serum of patients with DPN were significantly higher than those of patients without DPN (P <0.05) (P <0.05). The multivariate Logistic regression analysis of risk factors of DPN showed that ALT and γ ~ (-GT) were both type 2 diabetes mellitus patients with DPN Risk factors. Conclusions T2DM and DPN are related to the changes of liver function. Monitoring and controlling the levels of ALT and γ ~ (-GT) is beneficial to effectively intervene and improve the prognosis of DPN.
