目的探讨重型颅脑损伤伴有多发伤一体化治疗的临床效果。方法对2005年1月至2010年2月,应用“一体化”指导思想救治重型颅脑损伤合并多发伤(ISS≥25分)268例(A组)的临床资料进行回顾性分析,并与2001年1月至2004年12月收治的同类患者187例(B组)进行比较。结果 A组268例,死亡47例,占17.5%。B组死亡53例,占28.6%。A、B两组比较:ISS=25~45分的病例B组死亡率高于A组,特重型颅脑创伤组生存时间A组长于B组(P<0.05)。其中两组ISS评分:A组均分35.2,B组36.1,差异无统计学意义。结论迅速准确的伤情判断,积极地实行早期抢救、损伤控制,MODS分级防治的一体化治疗能够显著提高重型颅脑创伤合并多发伤生存率。
Objective To investigate the clinical effect of integrated treatment of severe traumatic brain injury with multiple trauma. Methods From January 2005 to February 2010, the clinical data of 268 patients with severe traumatic brain injury complicated with multiple trauma (ISS≥25 points) treated with the “integrated” guiding principle were retrospectively analyzed. Compared with 187 patients (group B) of the same type admitted from January 2001 to December 2004. Results A group of 268 cases, 47 patients died, accounting for 17.5%. 53 cases died in group B, accounting for 28.6%. The mortality of group B with ISS = 25-45 was higher than that of group A, and the survival time of group with special traumatic brain injury was longer than that of group B (P <0.05). Two groups of ISS score: A group of 35.2, B group 36.1, the difference was not statistically significant. Conclusions Rapid and accurate judgment of injury, active implementation of early rescue, injury control, integrated treatment of MODS grading and prevention can significantly improve the survival rate of severe traumatic brain injury combined with multiple injuries.
