目的探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者血清瘦素和甲状腺素水平的相关性以及两者在OSAHS病情进展中的作用。方法选择OSAHS患者(OSAHS组)和健康者(对照组)各40例,均为男性,分别行多导睡眠图检查,并测定空腹瘦素和甲状腺素水平。结果(1)OSAHS患者的血清瘦素、FT3以及颈围分别高于对照组(P<0.05)。且随着病情的加重,血清瘦素及FT3水平逐渐增加。(2)OSAHS患者血清瘦素水平与呼吸暂停低通气指数(AHI)、呼吸障碍事件最长时间(LAHD)、微觉醒指数(MI)均呈正相关(P<0.05),与最低血氧饱和度(MOS)呈负相关(P<0.05)。(3)OSAHS患者血清瘦素水平与FT3呈正相关(P<0.05)。(4)OSAHS患者FT3与AHI及MI均呈正相关(P<0.05),FT4仅与MI呈正相关(P<0.05)。(5)OSAHS患者FT3与颈围呈正相关(P<0.05),TSH与颈围呈负相关(P<0.05),TSH与脂肪百分比呈正相关(P<0.05)。结论(1)OSAHS患者的血清瘦素水平及颈围明显升高,且与病情严重程度相关。(2)OSAHS患者FT3浓度明显增高,且与颈围存在相关性,同时发现甲状腺素与OSAHS病情严重程度存在相关性。(3)OSAHS患者瘦素水平与FT3之间存在明显的相关性。
Objective To investigate the correlation between serum leptin and thyroxine in patients with obstructive sleep apnea-hypopnea syndrome (OSAHS) and their roles in the progression of OSAHS. Methods Forty OSAHS patients and 40 healthy volunteers (control group), all of whom were male, were examined with polysomnography, and the levels of fasting leptin and thyroxine were measured. Results (1) Serum leptin, FT3 and neck circumference of patients with OSAHS were higher than those of the control group (P <0.05). And with the aggravation of disease, serum leptin and FT3 levels gradually increased. (2) Serum leptin levels in OSAHS patients were positively correlated with AHI, LAHD, and MI (P <0.05), and were positively correlated with the lowest oxygen saturation (MOS) was negatively correlated (P <0.05). (3) The level of serum leptin in patients with OSAHS was positively correlated with FT3 (P <0.05). (4) There was a positive correlation between FT3 and AHI and MI in OSAHS patients (P <0.05), and FT4 only correlated with MI (P <0.05). (5) There was a positive correlation between FT3 and neck circumference in OSAHS patients (P <0.05). TSH was negatively correlated with neck circumference (P <0.05). TSH was positively correlated with fat percentage (P <0.05). Conclusions (1) Serum leptin level and neck circumference in patients with OSAHS are obviously increased, and are related to the severity of the disease. (2) The concentration of FT3 in patients with OSAHS was significantly higher than that in patients with OSAHS. There was a correlation between thyroid hormone and the severity of OSAHS. (3) There is a clear correlation between leptin level and FT3 in patients with OSAHS.
