Vit B_1缺乏症为可预防性疾病,脑型首累及视神经者罕有报道.现报告如下:1 临床资料3例患儿中,男2例,女1例;年龄15月、12月、11月各1例.3例均家住农村,母乳或加米粉喂养.1例病前半月因“口腔炎”当地误用 5%碳酸氢钠 300mL.静滴后昏迷,经抢救神志转清,但双目失明.3例患儿中,2例抽搐,其中1例双眼凝视3天,1例失明半月.伴发症:呼吸系统感染2例,消化系统感染1例.体检发现3例呆视,哭声嘶哑不显,四肢肌张力除1例抽搐间歇期较低外均正常,余无特殊阳性体征.辅助检查:血镁正常.血钙1例1.65mmol/L、2例正常.血丙酮酸568～85lμmmol/l.脑脊液常规、生化正常.脑电图表现为界限～正常.眼底检查:光瞬目反射消失,眼球运动障碍,光反应迟钝,眼底正常.诊断与治疗:均诊断脑型Vit B_1缺乏症.Vit B_1 0.1mg肌注,每日 1次,连续 3天.抽搐止,l～3天眼均复明.

Fundus examination: the disappearance of instantaneous reflection of the eye, eye movement disorders, photic retardation, fundus normal. Diagnosis and treatment: were diagnosed with brain-type Vit B 1 deficiency .Vit B_1 0.1mg intramuscular injection, 1 day, for 3 days. Twitch only, l ~ 3 days are complex eyes.
