目的:探讨上颌前方牵引矫治骨性Ⅲ类错软硬组织A点和B点矢状向垂直向的变化及其相关性。方法:选择20例骨性Ⅲ类错患者,采用上颌前方牵引结合快速扩弓进行矫治,对治疗前后的软硬组织头影测量值进行配对t检验、直线相关分析。结果:在矢状向上,硬组织点A与软组织点A(sA)前移的比例为1∶0.87,硬组织点B与软组织点B(sB)后退的比例为1∶0.96;在垂直向上,A与sA和B与sB分别下移的比例分别为1∶1.09和1∶1.21。A与sA(矢状向r=0.612,垂直向r=0.459,P<0.01)、B与sB(矢状向r=0.885,垂直向r=0.689,P<0.01)的移动呈现密切的正相关关系,唇部形态随着软硬组织A、B点的变化而趋于协调。结论:上颌前方牵引矫治骨性Ⅲ类错软硬组织A点和B点在矢状向垂直向的变化呈正相关,且下颌的反应较上颌更加明显。
OBJECTIVE: To investigate the changes of vertical and sagittal vertical distribution of point A and point B in skeletal Class Ⅲ malocclusion treated with maxillary anterior traction. Methods: Twenty patients with skeletal class Ⅲ malocclusion were selected and treated by maxillary anterior traction combined with rapid protraction. Paired t-test and linear correlation analysis were performed on the measured values of the cephalocele before and after treatment. Results: In sagittal direction, the ratio of hard tissue point A to soft tissue point A (sA) was 1: 0.87, soft tissue point B and soft tissue point B (sB) were 1: 0.96, and in vertical direction, The proportions of A and sA and B and sB down were 1: 1.09 and 1: 1.21, respectively. There was a close positive correlation between B and sB (sagittal r = 0.885, vertical r = 0.689, P <0.01) with A and sA (sagittal r = 0.612, vertical r = 0.459, P <0.01) Relationship, lip morphology with the hard and soft tissue A, B points tend to be coordinated. Conclusion: There is a positive correlation between the sagittal and vertical changes of points A and B in skeletal Class Ⅲ malocclusion treated by anterior maxillary distraction, and the response of mandible is more obvious than that of maxillary.
