根据100例经鼻蝶窦手术患者的头颅X线平片、矢状断层和冠状CT扫描片,以及手术中直接观察的结果,仔细研究了蝶窦的气化类型,提出一种新的两侧组合分型法。发现相同型蝶窦占58%,不同型占42%,前者进一步分为5个亚型,后者可分为10个亚型。强调此种分型法对经鼻蝶窦手术时选择合适的进路具有重要意义。蝶上筛房的出现率为22%,如果手术者不熟悉它的解剖,术中易将其误认为蝶窦,而误伤颈内动脉和视神经等。
