目的:探讨活动期和静止期狼疮性肾炎(LN)患者外周血中调节性T细胞CD4+CD25+CD127lo(Treg)变化的意义。方法:流式蛋白分析系统(CBA)测定外周血IL-6、IL-10水平,流式细胞仪测定外周血CD19+CD69+、CD19+凋亡率、Tregs水平。结果:(1)①LN患者CD19+CD69+、CD19+凋亡率水平较正常组升高(P<0.01);②活动期LN CD4+、CD4+CD25+CD127lo水平较正常组降低(P<0.01);静止期CD4+、CD4+CD25+CD127lo与正常组比较差异无统计学意义;③活动期LN患者IL-10、IL-6、CD19+CD69+、CD19+凋亡率水平较静止期显著升高(P<0.01)。(2)Treg与CD19+CD69+、CD19+凋亡率无相关性,与IL-10、IL-6呈负相关(P<0.05),与ds-DNA、SLEDAI呈负相关(P<0.01)。CD19+凋亡率与IL-10呈负相关(P<0.01),与SLEDAI、CD19+CD69+呈正相关(P<0.01)。结论:Treg可作为狼疮活动性的指标,并能够减少或完全抑制自身抗体的产生。
Objective: To investigate the significance of the changes of CD4 + CD25 + CD127lo (Treg) in peripheral blood of patients with lupus nephritis (LN) during active and quiescent periods. Methods: The levels of IL-6 and IL-10 in peripheral blood were measured by flow cytometry (CBA). The levels of CD19 + CD69 +, CD19 + apoptosis and Tregs in peripheral blood were measured by flow cytometry. Results: ①The levels of CD19 + CD69 + and CD19 + apoptosis in LN patients were significantly higher than those in normal controls (P <0.01); ②The levels of LN CD4 +, CD4 + CD25 + CD127lo in active LNs were lower than those in normal controls (P <0.01) There was no significant difference in the level of CD4 +, CD4 + CD25 + CD127lo compared with the normal group. ③The apoptotic rate of IL-10, IL-6, CD19 + CD69 + and CD19 + ). (2) There was no correlation between Treg and CD19 + CD69 +, CD19 + apoptotic rate, but negatively correlated with IL-10 and IL-6 (P <0.05). The apoptosis rate of CD19 + was negatively correlated with IL-10 (P <0.01), and positively correlated with SLEDAI, CD19 + CD69 + (P <0.01). Conclusion: Treg can be used as an indicator of lupus activity and can reduce or completely inhibit the production of autoantibodies.
