目的探讨彩色多普勒超声(CDFI)血流分级及阻力指数(RI)在乳腺良、恶性肿块鉴别诊断中的应用价值。方法选取2014年10月—2015年8月景德镇第二人民医院就诊的女性乳腺肿瘤患者160例。根据病理检查结果不同分为恶性组89例与良性组71例。分析两组患者CDFI血流分级、收缩期流速峰值(PSV)及RI值情况。结果良性组肿块血流信号检出率为54.9%(39/71),恶性组肿块血流信号检出率为96.6%(86/89);恶性肿块以Ⅰ~Ⅲ级血流为主,占96.6%(86/89),良性肿块以0~Ⅰ级血流为主,占84.5%(60/71);两组患者CDFI血流分级比较,差异有统计学意义(P<0.05);恶性组患者PSV、RI值高于良性组,差异有统计学意义(P<0.05)。结论 CDFI对乳腺肿块的检出率高,CDFI血流分级及RI值可提高乳腺癌患者乳腺良、恶性肿块的鉴别诊断水平。
Objective To investigate the value of color Doppler flow imaging (CDFI) and resistance index (RI) in the differential diagnosis of benign and malignant breast tumors. Methods A total of 160 female breast cancer patients from the Second People’s Hospital of Jingdezhen from October 2014 to August 2015 were selected. According to the results of pathological examination were divided into malignant group 89 cases and 71 cases of benign group. CDFI blood flow classification, peak systolic velocity (PSV) and RI values of two groups were analyzed. Results The detection rate of blood flow signal was 54.9% (39/71) in benign group and 96.6% (86/89) in malignant group. The grade Ⅰ ~ Ⅲ blood flow in malignant mass was the main component, accounting for 96.6% (86/89) of benign masses were mainly grade 0 to grade Ⅰ, accounting for 84.5% (60/71). There was significant difference in CDFI grade between the two groups (P <0.05) The PSV and RI values of the patients in the group were higher than those in the benign group, the difference was statistically significant (P <0.05). Conclusion The detection rate of CDFI on breast tumor is high, CDFI grade and RI value can improve the differential diagnosis of benign and malignant breast tumors in breast cancer patients.
