目的 研究小肠腺瘤和腺癌组织中碱性成纤维细胞 (bFGF) ,血小板衍生生长因子(PDGF)和血小板反应蛋白 1(TSP 1)三种mRNA表达水平及其临床病理意义。方法 收集小肠腺癌手术标本 44例 ,病理类型包括腺癌Ⅰ、Ⅱ、Ⅲ级各 11、2 6、7例 ;癌细胞未侵犯肌层或仅达浅肌层 12例 ,侵犯深肌层 18例 ,浆膜层 14例。 10例良性病变包括十二指肠腺瘤 9例和回肠腺瘤 1例。三种mRNA染色方法均为原位杂交染色法。结果 44例小肠腺瘤bFGFmRNA、PDGFmRNA和TSP 1mRNA阳性率分别为 5 7% ,48%和 36 % ,10例小肠腺瘤则分别为 2 0 % ,30 %和 90 %。小肠腺瘤bFGFmRNA阳性率明显高于腺瘤 (P <0 .0 5 ) ,而TSP 1mRNA则相反 (P <0 .0 5 )。腺癌Ⅰ级 +Ⅱ级、癌细胞未侵犯肌层 +侵犯浅肌层、肿块最大径 <3cm和未见淋巴结转移病例 ,bFGFmRNA、PDGFmRNA阳性率明显低于腺癌Ⅲ级、癌细胞侵犯深肌层 +浆膜层、肿块最大径≥ 3cm和淋巴结转移病例 ,但TSP 1mRNA表达则相反。bFGFmRNA在腺癌中表达与PDGFmRNA表达呈正相关 ,但与TSP 1mRNA表达呈负相关。结论 bFGF、PDGF和TSP 1均为重要的肿瘤血管生成调节因子 ,其中bFGF、PDGFmRNA表达与小肠癌发生、发展、生物学行为、转移及其预后有密切关系 ,为重要生物学标记物 ;而TSP 1mRNA表达者可能分化?
Objective To study the expression of basic fibroblast growth factor (bFGF), platelet derived growth factor (PDGF) and thrombospondin 1 (TSP 1) in intestinal adenoma and adenocarcinoma and its clinicopathological significance. Methods Forty-four cases of small bowel adenocarcinoma were collected. The pathological types included 11, 26, and 7 cases of adenocarcinoma Ⅰ, Ⅱ and Ⅲ, respectively. The cancerous cells did not infiltrate the muscular layer or only the superficial muscular layer in 12 cases, Cases, serosa in 14 cases. Ten cases of benign lesions including duodenal adenoma in 9 cases and ileal adenoma in 1 case. Three mRNA staining methods are in situ hybridization staining. Results The positive rates of bFGFmRNA, PDGFmRNA and TSP 1mRNA in 44 cases of small intestinal adenomas were 57%, 48% and 36% respectively, while those in 10 cases were 20%, 30% and 90% respectively. The positive rate of bFGF mRNA in intestinal adenomas was significantly higher than that in adenomas (P <0.05), while TSP 1 mRNA was opposite (P <0.05). The positive rates of bFGFmRNA and PDGFmRNA were significantly lower than those of adenocarcinoma grade ¢ ó and adenocarcinoma of grade ¢ òand grade ¢ ò. The cancerous cells did not infiltrate the myometrium and superficial myometrium. The maximum diameter of tumor was less than 3 cm and no lymph node metastasis. Layer + serosa, the largest diameter of tumor ≥ 3cm and lymph node metastasis, but TSP 1mRNA expression is the opposite. The expression of bFGF mRNA in adenocarcinoma was positively correlated with PDGF mRNA expression, but negatively correlated with TSP 1 mRNA expression. Conclusions Both bFGF, PDGF and TSP 1 are important tumor angiogenesis regulators. The expressions of bFGF and PDGF mRNA are closely related to the occurrence, development, biological behaviors, metastasis and prognosis of small intestine cancer and are important biomarkers.
