由于钙离子在心肌细胞兴奋-收缩偶联中发挥着核心作用,低钙血症可诱发心肌病。临床上,由于低钙血症导致的T波电交替和急性心衰罕见,极易被忽视或误诊。研究显示,低钙血症引起的T波电交替和急性心衰往往是可逆的,因而及时纠正严重低钙血症十分重要。本文报告一例慢性甲状旁腺功能减退症患者,由于严重的低钙血症引起T波电交替和急性心衰发作,在迅速纠正低钙血症后,患者病情得以缓解,T波电交替完全消失,心功能部分恢复。通过复习该病例,有助于临床医生重视可引起T波电交替和心衰的可逆性因素—低钙血症。

Clinically, T-wave alternans and acute heart failure due to hypocalcemia are rare and easily overlooked or misdiagnosed. Studies have shown that T-wave alternation and acute heart failure due to hypocalcemia are often reversible, and it is important to promptly correct severe hypocalcemia. This article reports a case of chronic hypoparathyroidism patients, due to severe hypocalcemia caused by alternating T-wave power and acute heart failure, the rapid correction of hypocalcemia, the patient’s condition has been alleviated, T wave alternating completely disappeared Heart function recovered partially. By reviewing this case, it helps clinicians value the hypocalcemia, a reversible factor that causes alternating T waves and heart failure.
