作者所在医院近20年收治564例心内膜炎,其中27例伴脾脓肿,计男18例,女9例,平均37岁(22～73岁)。症状有发热25例,呼吸困难10例,左胸膜病6例,长期厌食6例。入院时6例发现有充血性心衰,3例有明显肺炎。27例均有瓣膜赘生物,均接受静脉抗生素治疗,平均治疗期限为5.6周。24例血培养阳性,主要菌株是金黄色葡萄球菌(16例)。12倒脾组织细菌培养阳性。1980年前,细菌性心内膜炎併发脾脓肿均在尸检时确诊。1980年后,则可经CT扫描证实。
The author’s hospital admitted 564 cases of endocarditis in the past 20 years, including 27 cases with splenic abscess, 18 males and 9 females, average 37 years (22-73 years). Symptoms of fever in 25 cases, 10 cases of dyspnea, left pleurisy in 6 cases, long-term anorexia in 6 cases. 6 cases admitted to hospital found congestive heart failure, 3 patients had obvious pneumonia. All 27 patients had valvular vegetations and received intravenous antibiotics, with an average duration of 5.6 weeks. 24 cases of blood culture positive, the main strain is Staphylococcus aureus (16 cases). 12 inverted spleen tissue culture positive. By 1980, bacterial endocarditis complicated with splenic abscess were confirmed at autopsy. After 1980, it can be confirmed by CT scanning.
