目的对猫脑挫裂伤周围半暗带的影像学及超微结构进行动态研究。方法采用改良的Feeney法制作猫脑挫裂伤模型,伤后1h用超导3.0T磁共振扫描仪行磁共振扫描,观察挫裂伤周同影像学变化,并进行定位取材,行光镜和透射电镜检查。结果MRI可见挫裂伤周围脑区呈低灌注表现,DWI上低灌注区明显小于PWI上低灌注区,超微结构可见神经元、星形细胞、血脑屏障不同程度的损伤,神经元凋亡现象,并与时间和空间呈相关性。结论挫裂伤灶周围确实存在“创伤半暗带”;其发生发展过程及临床诊治需进一步研究。
Objective To study the imaging and ultrastructure of the penumbra around cerebral contusion and laceration in cats. Methods A modified Feeney method was used to make cat brain contusion and laceration model. The superconducting 3.0T magnetic resonance scanner was used to perform MRI scans at 1h after injury. The imaging changes of contusion and laceration were observed, And transmission electron microscopy. Results MRI showed low perfusion in the brain area around the contusion and laceration. The hypoperfusion area on DWI was significantly lower than that on the perfusion area on PWI. Ultrastructure showed neurons, astrocytes, different degree of damage on the blood-brain barrier and neuronal apoptosis Phenomenon, and with time and space were related. Conclusion There is indeed a “traumatic penumbra” around the contusion and laceration lesion. The development and clinical diagnosis and treatment of it need further study.
